This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. MONTANA NATIONAL GUARD W9124V-11-T-3001 REQUEST FOR QUOTATION FOR YOUTH CHILD CARE SERVICES FOR THE MONTANA ARMY NATIONAL GUARD YELLOW RIBBON PROGRAM 01 January 30 September 2011 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in the Federal Acquisition Regulations (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The NAICS code is 624410 and size standard is less than $7.0 million. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-26, Effective 1 June 2008 & Class Deviation 2005-00001. PERFORMANCE WORK STATEMENT This Performance Work Statement (PWS) will provide Youth child care services at the conference center or other contracted facility for children ages 6-18 in support of the Montana Army National Guard (MTARNG) Yellow Ribbon program. Typically, these events are held on a monthly basis at different locations in the state of Montana. Locations in Missoula, Helena, Great Falls, and Billings are not unusual. Contractor shall provide a copy of a care license issued by the state of Montana as well as CPR certificates and proof of favorable background checks for staff. Contractor must provide proof of insurance for an offsite location with children not enrolled in their day care curriculum. The staff under this contract shall provide a sample curriculum with their proposal. The purpose of the curriculum is to occupy the youth children with crafts, activities, music and reading. Contractor is responsible for child care related items to include toys, books, craft materials, cleaning materials, hygienic materials, and other small item supplies needed for the care of youth between the ages of 6 and 18 year olds up to state standards for licensed day care. The contractor is responsible provide transportation, lodging and per diem for their staff to the event. Contracting Officers Representative (COR) will provide contractor the facility the where the event will be located no later than five (5) days prior to the event. Offeror shall provide a firm, fixed price for each event in their proposal. Proposal shall also include proof of insurance, proof of license issued by the state of Montana, proof of Infant CPR certification and proof of favorable background check. Proposal shall also include sample curriculum conforming with the statement of work. Finally, proposal shall include no more than five (5) examples of child care services provided at an offsite location with a magnitude between five (5) and thirty (30) children. Offerors shall provide a client point of contact for each example with name and telephone number. Offeror should be advised that upon request, Operation Military Kids (OMK) will make a good faith effort to find volunteers and partners to provide OMK curriculum, Hero Packs and Mobile Technology Lab activities at Yellow Ribbon events. OMK will collaborate with other partners to train volunteers on the OMK Yellow Ribbon Curriculum and Mobile Technology Lab activities. All offers will be considered based upon the lowest price technically acceptable and best value to the government. The provision of 52.212-3 Alt I, Offeror Representations and Certifications-Commercial Items (April 2002) applies to this RFQ. 52.212-3 Alt I clause needs to be completed and a signed copy of this provision shall be submitted with any offer. 52.212-2, Evaluation Commercial Items (Janaury 1999). The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors are considered. The following factors shall be used to evaluate offers: Price and Technical Capability. If products from offerors are found to be technically equivalent, price will be the determining factor.
