Revised Attachment 4 'Past Performance Questionnaire' to reflext extension of deadline for submittal. New Pages 59, 62, 68 SF30 HSHQDC-11-R-00007 A00002 ********************************** AMENDMENT A00001 10 November 2010 ********************************** The purpose of Amendment A00001 is to: 1) Extend the deadline for proposal submission from November 17, 2010 to December 01, 2010 at 4:30 Eastern Time. 2) Extend the deadline for Past Performance Questionnaires from November 08, 2010 to December 01, 2010 at 4:30 Eastern Time. 3) As a result of the deadline extension, pages 59, 62, and 68 are hereby updated (see attachments). and 4) Replace Attachment 4 "Past Performance Questionnaire" with the attached Revised Attachment 4 to reflect deadline change. Note: This change does not require re-submittal of previously submitted questionnaires. Note: Questions and Answers for this solicitation will be posted no later than Tuesday, November 16, 2010. ************************************ ************************************
