Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The NAICS code is 621330 and size standard is less than $7.0 million. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-26, effective 1 June 2008 & Class Deviation 2005-00001. Performance Work Statement 1. Background. The Montana National Guard has invested a tremendous amount of resources to enhance its Deployment Cycle Support program. One of these enhancements is the periodic behavioral health screenings of service members during Individual Drill Training (IDT) weekends following deployment. Screening is now mandatory every six months for two years after deployment; after two years, screening is mandatory once per year. The Montana National Guard is not resourced with enough credentialed behavioral health care providers to conduct all the necessary behavioral screenings and has a requirement for weekend behavioral health providers. The annual screenings are to be conducted in conjunction with the annual Periodic Health Assessment (PHA) process. 2. Description of Services. The Montana National Guard anticipates contracting with a single contractor for behavioral healthcare professionals who shall be capable of: (a)Conducting behavioral health care screenings of Soldiers and Airmen, which will: (1) Be face-to-face screenings conducted on IDT (drill) weekends. (2) Utilize Montana National Guard screening/referral forms (attached). (3) Be conducted at a rate of up to 10 Soldiers/Airmen per hour per provider (80 per day) depending on screening population (4) Be conducted by properly credentialed health care providers. (b)Providing educational talks and presentations. (c)Discussing behavioral health issues one-on-one with Montana Soldiers and Airmen. (d)Facilitating a professional working relationship with the Montana National Guard medical staff. 3. Requirements for Health Care Providers. To be provided post-award: (b) Professional credentials and forms, to be given to the Air National Guard 120th MDG/SGA Credentials Manager, which shall be the following: - A current, unrestricted license to practice as a Licensed Clinical Practicing Counselor (LCPC) or a Licensed Clinical Social Worker (LCSW) in the State of Montana; - Evidence of malpractice insurance from a property casualty insurer licensed in the State of Montana; - National Practitioner Identifier; - Current Basic Life Support (BLS) documentation - A diploma, - Records of CPE (Continuing Professional Education) for the past 24 months - AF Form 1540, Application for Clinical Privileges/Medical Staff Appointment; - AF Form 1562, Credentials Evaluation of Health Care Practitioners; - AF Form 1541, Credentials Continuing Health Education Training Record 4. Deliverables. (a) Screening and referral forms: To be submitted to Medical Detachment (MED DET) and Medical Group Officer in Charge at the conclusion of each IDT weekend. (b) Monthly reports: (1) To the Montana Army National Guard Office in Charge summarizing services provided; (2) To the Montana Air National Guard Office in Charge summarizing services provided; and (3) To the Montana National Guard JFHQ J-1 summarizing services provided, to include a summary of referrals and any other information necessary to evaluate the effectiveness and value of the services provided. The monthly report to the J-1 shall not contain PHI (personally identifiable healthcare information). (c) An annual report: (1) To the Montana Army National Guard Office in Charge summarizing services provided; (2) To the Montana Air National Guard Office in Charge summarizing services provided; and (3) To the Montana National Guard JFHQ J-1 summarizing services provided, to include a summary of referrals without PHI (personally identifiable healthcare information), and any other information necessary to evaluate the effectiveness and value of the services provided. The annual report to the J-1 shall not contain PHI (personally identifiable healthcare information). 5. Schedule of Events, Base Year. Dates for Option Years 1-4 will be provided with Notice of Intent to Exercise Option, no later than 60 days prior to the effective date of the option. The total potential effective period of the award resulting from this solicitation is is 60 months from date of award. The Contractor shall not be required to accept any new orders beyond 60 months from the award date. Quality Assurance Surveillance Plan This Quality Assurance Surveillance Plan (QASP) has been developed and designed by the Contracting Officers Representatives (CORs) at Fort Harrison, MT for Behavioral Health Screening Services Contract, Contract. This contract will be administered through the J1 Director of Manpower and Personnel at Fort Harrison Montana and Gore Hill Great Falls Montana and is designed to provide behavioral health care screening for the members of the Montana Army and Air National Guard The quality of the services provided will be assessed and surveilled in the following three areas: 1.Assurances prior to screenings: a.Surveillance of provider credentials and required forms (current and complete). b.Surveillance that proper badges and building access coordination have been made. c.Surveillance that a private, privacy sound-proof, clinic exam room is available in which to conduct the screenings. d.Surveillance to ensure that HIPPA Business Associate agreements have been completed with any non-provider organizations who may be receiving or processing PHI related to these screenings. e.Periodic surveillance of National Healthcare Provider Data base for reportable information on contracted health care providers. f.Surveillance of provide travel arrangements (checking to see if travel and lodging arrangements are available) 2.Assurances during screenings: a.Planning and surveillance to ensure that pre-screening forms are able to be filled out privately by soldiers, that sufficient time is given to soldiers to fill out forms, and that clip boards and pencils are provided. b.Planning and surveillance to ensure that pre-screening forms are individually stapled or prong-clipped inside a privacy file folder so that privity of the disclosed information is maintained while the soldiers are waiting to see their health care providers. c.Planning and surveillance to ensure that anyone referred to the PHA screeners who is not going through the entire health assessment is wearing the same uniform as those going through the entire health assessment. d.Surveillance to ensure that appropriate patient flow is maintained through the screening station. e.Surveillance of reports turned in by the health care providers at the end of each IDT weekend, assuring timely reporting and completeness of the forms submitted. 3.Assurances associated with payment a.Review of each monthly invoice and assurance that billed services were properly provided and all deliverables have been submitted, inspected, and accepted. b.For monthly invoices, submission of a written recommendation to the Contracting Officer for each invoice recommending rejection or payment of submitted invoices. c.For final payment, submission of a written recommendation to the Contracting Office stating that all contract services and deliverables, including the required annual report, has been submitted, inspected, and accepted. Additional Information COR: The Contracting Office shall appoint a COR to assist in the administration of this contract and its associated Quality Assurance Surveillance Plan. A sample appointment letter is attached. Quality Assurance: The contractor shall adopt a quality assurance program that monitors all service activities to ensure contract compliance. The government will monitor the contractors performance in accordance with the Quality Assurance Surveillance Plan. The QASP is attached. Regulatory Compliance: The contractor shall ensure the requirements of Occupational Safety and Health Administration (OSHA), HIPPA, and other applicable Federal regulations are met. Security Requirements: All contract employees shall adhere to all local and National Guard security regulations and directives. Invoicing and Payment: This contract will have separate CLINs for each drill weekend, by location. A separate invoice may be submitted for each CLIN, for which separate payment will be made. Advance payments for services will not be made. Final payment, after the last IDT weekend of the year, will be authorized only after all services, including submission, inspection, and acceptance of the required annually summary report, has been completed. Self-referrals: Referral of a soldier for additional treatment to a medical group affiliated with the referring provider is prohibited by this contract. Additionally, self-referrals are prohibited by federal rules against fraud, waste and abuse. Should a self-referral be discovered it will be a reportable event and grounds for contract termination. Referrals: TRICARE is the Department of Defenses health care service provider. TRICAREs Western Regional Service Provider is TriWest Healthcare Alliance. All referrals for additional treatment must be made to TriWest Healthcare Alliance mental healthcare providers. Federal Funding: Funds are not presently available for this acquisition. Proposals shall also include no more than 3 past performance narratives describing past performance of services of similar scope as described in the performance work statement. Proposal shall also include technical approach narrative that addresses the contractors ability to meet the requirements in the above Performance Work Statement and the contractors experience with and knowledge of Army National Guard behavior health operations and regulations. Contractor is authorized travel and per diem in accordance with the JTR. 52.212-2, Evaluation-- Commercial Items (January 1999). The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the government. The following factors shall be used to evaluate offers: price, past performance, and the following technical factors in descending order of importance: 1) The contractors ability to meet the requirements in the above Performance Work Statement; and 2) Contractors experience with and knowledge of Army National Guard behavior health operations and regulations. The technical evaluation factors listed above, and past performance, when combined are approximately equal in value to Price.
