- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number FM285502520002 is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-43. Applicable NAICS is 33112; size standard is 500 employees. The Government intends to make a sole source award to Medical Education Technologies, Inc. (METI) 6300 Edgelake Drive, Sarasota FL 34240-8817. Requirement is to purchase 2 each HPS manikins, PN HPS-010 with the following accessories: GAS-002, Gas Accessory Kit-Anesthesia, HPS-300, Anesthesia Delivery System (Standard), and MON-001, Monitor Interface Kit. Shipping address is 711 HPW/OML, 2415 Sidney Brooks Road, Brooks-City Base TX 78235-5119. FOB: Destination. A firm fixed price (FFP) contract will be issued. All responsible offers will be considered; however, offers must be submitted for the item listed above. Substitutes are not acceptable. The following provisions are applicable: 52.212-1, Instructions to Offerors - Commercial; 52.212-2, Evaluation - Commercial Items; 52.212-3, Offeror Representations and Certifications - Commercial Items; 52.212-4, Contract Terms and Conditions - Commercial Items; 52.212-5, Contract terms and Conditions Required To Implement Statutes Or Executive Orders - Commercial Items.
