HQ0034-10-T-0091 52.212-2 Addendum HQ0034-10-T-0091 Contractor Questionnaire HQ0034-10-T-0091 Solicitation HQ0034-10-T-0091 WDR HQ0034-10-T-0091 Performance Work Statement This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is HQ0034-10-T-0091; this solicitation is a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-45. This acquisition will be a 100% total small business set-aside; the North American Industry Classification System (NAICS) code is 562213. The business size standard is $12,500,000. The Federal Supply Class (FSC) is J099. The Washington Headquarters Service, Acquisition and Procurement Office (WHS/A&PO) Department of Defense, 1155 Defense Pentagon, Washington, DC 20301 intends to purchase Incinerator Support Services to provide for the operation, maintenance, and repairs for the Incinerator Facility at the Pentagon. The Incinerator Facility is a Top Secret (TS) facility and will require the potential awardee to already have and maintain TS clearance. The DD254 will be attached as an addendum to this synopsis/solicitation by November 1, 2010. The Government anticipates awarding one firm fixed price contract for this requirement. This request for quotation consists of the CLINs defined in solicitation HQ0034-10-T-0091 Schedule B. Schedule B identifies the period of performance for the base period and subsequent option periods. See solicitation HQ0034-10-T-0091 for the performance work statement and more information regarding this requirement. See solicitation HQ0034-10-T-0091 52.212-2 Addendum for proposal requirements and evaluation criteria. Additional Information: 1. ONLY e-mailed quotes will be accepted. All responsible sources interested in responding to this RFQ must respond no later than 10:00 AM Eastern Standard Time on November 22, 2010. No phone communication will be accepted. 2. Procedures under Federal Acquisition Regulations, Subpart 13 Simplified Acquisition Procedures are applicable to this RFQ. The Government will evaluate quotes and make a best value selection based on the tradeoff process. The Government is not responsible for locating or obtaining any information not identified in the offer. 3.
