- SOURCES SOUGHT NOTICE Please note that this is NOT a request for SF330's qualification packages or proposals. The Government is seeking information for market research purposes only to determine if there are sufficient Architect-Engineering firms within the socioeconomic categories designated in Federal Acquisition Regulation Part 19.1007(b)(2) to merit a set-aside in this order of preference: 1) Service Disabled Veteran Owned Small Business; 2) 8(a) Program; 3) HUBZone Program. The Government may or may not issue additional announcements or solicitation documents. Any respondent to this market survey must be a well qualified small business registered AE firm with the necessary professional qualifications, technical competence, specialized experience, capacity, satisfactory past performance, and have a working office located within within a 150 mile radius of the project location in one of the aforementioned socioeconomic categories. The A/E services outlined below are required for the Department of Veterans Affairs Aleda E. Lutz VA Medical Center located in Saginaw MI. Applicable NAICS code is 541330, Small Business Size Standard is $4.5 million average annual gross receipts the preceding 3 years. In accordance with Veterans Affairs Acquisition Regulation 836.204, the estimated construction cost of the project resulting from the design is expected to be between $1,000,000 and $2,000,000. Description of work to be performed includes: The Architect/Engineer firm shall provide all labor and supervision necessary to accomplish the following design work to provide Schematic Documents, Design Development Documents, Construction Documents, Bid Documents, Technical Specifications, Construction Period Services, Site Visits, Cost Estimates, As-Built Documentation, and all other related information for project #655-10-123 entitled Design Patient Care Center. 1.Design Intent: The intent of this project is to prepare plans and specifications necessary to implement the model of Patient Aligned Care Team in three separate primary care clinics which includes green team, red team, and blue team at the Aleda E. Lutz VA Medical Center, Saginaw, Michigan in accordance with the following. A.General Scope of the Project: The Architect / Engineer (A/E) will verify record drawings and existing conditions; prepare fully developed Construction Documents (drawings and specifications); construction phasing (scheduling) timeline; and cost estimates. B.The total completed design shall include all necessary features and items of work required so that the government shall have a complete buildable and functional system complying with the latest requirements of the Department of Veterans Affairs design criteria and all applicable national codes and standards. It is anticipated that the design effort will involve mechanical, electrical, architectural, plumbing and structural disciplines. C.Provide all drawings, specifications, equipment selection, equipment layout, cost estimate, including but not necessarily limited to architectural, structural, plumbing, mechanical, and electrical features as required for this contract. 2.Design Effort: A.Building 1 Patient Aligned Care Team (PACT) a.Analyze the extent of demolition and modifications to existing areas and/or components and design the renovation of the selected areas. b.Incorporate into design provider and nursing offices in the team along with space for social workers, pharmacist, dietician, and Clerks with an efficient work flow. c.New items included but not limited to the following: Design all related architectural, plumbing, structural, mechanical, controls and electrical features required for a complete system. d.PACT is a team-based model of care led by a primary care provider who is supported by a fully engaged and collaborative team. The team works together to provide coordinated and comprehensive care throughout the patient ™s lifetime in order to maximize health outcomes. The team is responsible for all of the patient ™s health care needs. The principles of a medical care home emphasize the personal relationships, team delivery of care, care coordination across specialties and settings of care, quality and safety, open access and affordable care. e.Design large exam rooms that can fit families and where computer workstation configuration permits eye to eye contact. Allow any necessary equipment to either be stocked in the room or be wheeled in on a cart so the patient never has to move. The patients should only have to go two places, the reception area and the exam room. Avoid specialized area, the staff should come to the patient in the exam room. Vitals and blood draws are done in the exam room. Responses shall be submitted via an email response and received no later than close of business Oct 29, 2010. No additional information regarding the scope of work is available at this time. No telephone or fax inquires will be accepted. All responses to this notice must identify the socioeconomic category of the respondent, contain a statement the firm is well qualified to accomplish the design based on the scope of work and the qualifications criteria noted above, is a registered professional Architect and/or Engineering design firm (identify state so registered), and provide the name and address of the firm, name and phone number of contact person, and the company's DUNS number. The respondent must be registered in the Central Contractor Registration (www.ccr.gov) and the Online Representations and Certifications website (orca.bpn.gov) and must indicate the proper socioeconomic status category(s) therein. If a Service Disabled Veteran Owned Small Business, the respondent must also be registered in the Vetbiz Registry (vip.vetbiz.gov/). Interested firms will be required to monitor and respond to additional announcements SEPARATELY from this announcement without further notice from the Government when posted to Federal Business Opportunities (www.fbo.gov/).
