- DEPARTMENT OF VETERANS AFFAIRS Justification and Approval For Other Than Full and Open Competition 1. Contracting Organization: Department of Veterans A IThirs, VISN 08, VA San Juan Medical Center Description of Action: Only One Responsible Source and No Other Supplies or Services Will Satisfy Agency Requirements. Description of Supplies or Services: Supply pharmacy waiting area furniture in concurrence and maintaining continuity with the furniture program. 2. Statutory Authority: (x) (1) Only One Responsible Source and No Other Supplies or Services Will Satisfy Agency Requirements; 3. Validity of Use of Statutory Authority: 6.302-1(4) When the agency head has determined in accordance with the agency ™s standardization program that only specified makes and models of technical equipment and parts will satisfy the agency ™s needs for additional units or replacement items, and only one source is available. Pharmacy Waiting Area is one of the largest waiting spaces available inside the VA San Juan Hospital. As part of a design initiative to maintain continuity on: Chair design, Fabric colors and textures. Our evaluation considers also the following specifications: Finishes: Fabric: Combo-S:MAH CHIME MOREL-B:MAH WHIRL SHELTER Wood Finish: GIN-GINGER- Patient Care and Infection Control: Considering the patients that would use this furniture Integra provide FABRIC GRADE R-Yds:4.3 and vinyl seats in compliance with infection control. This specification is also offered for bariatric patients. Warranty: Integra offers a lifetime warranty. This supports the inmense use this kind of furniture will have Integra Furniture is required in order to supply Pharmacy Waiting Area Chairs. 4. Actions to Promote Competition: The agency has determined in accordance with the agency ™s standardization program that only specified makes and models of technical equipment and parts will satisfy the agency ™s needs for additional units or replacement items, and only one source is available 5. Determination of Fair and Reasonable Price: The anticipated price to the Government is S. 113,409.93. Prices are fair and reasonable in accordance with catalog prices and previuos procurements. 6. Market Survey (FAR 10): A furniture supplier evaluated with the above specifications and not complies with the design intent inside the VA San Juan Hospital: 1. Brandrud 2. MA Esteves 3. Nameshof 7. Other Considerations: Ensures integrity and continuance of our furniture program. 8. Listing of Sources that expressed, in writing, an interest in the acquisition: 9.
