Description
Service, Material and Labor for on-site respirator fit testing at the Poplar Bluff VA Medical Center and five Medical Community Based Outpatient Clinics as outlined in the attached statement of work. STATEMENT OF WORK Poplar Bluff VAMC has a requirement for Fit Testing and Respirator Training for its employees. Contractor shall provide all material, manpower, and supplies (other than the respirator) to conduct qualitative respirator fit testing for any of the following: N95 filtering face-piece respirator, elastomeric half face-piece cartridge respirator or powered air purifying respirators (PAPR) (to be supplied by the VAMC) in compliance with all applicable regulations including but not limited to the OSHA Respirator Standard and respirator training to VA employees that the Contractor has fit tested. The Poplar Bluff VAMC will conduct a medical evaluation and the employee must present the Contractor with the Respirator Medical Clearance form signed by the physician or licensed health care provider in order to be fit tested or trained (Attachment A). Poplar Bluff VMCA anticipates Fit Testing and Respirator Training for approximately 550 employees. Training requirements include: Respirator fit testing Proper use of respirators in routine and emergency situations Procedures and schedules for cleaning, disinfecting, storing, inspecting, repairing, discarding and maintaining respirators Proper use of respirators to include putting on, removal and limitations Respiratory hazards recognition. Training requirements for PAPR include: Visual check of the hose to ensure there are no holes, tears, or Visual check of personal head cover to ensure there are no holes, tears, or rips Turn the battery pack on (disconnect from charger if necessary), ensure the hose is attached and place the air flow indicator at the end of the hose. It should hover at the top of the air hose. If both lines on the air flow indicator are not visible there is not adequate air flow to maintain positive pressure and the PAPR should not be used (most likely due to inadequate charge or loaded filter). Adjust the belt and battery pack assembly to fit snugly around waist. Snap the hose into the head cover and place the head cover around the face and over the head. Following use, return unit to location, wipe down with cavi-wipe and place back on charge. Maintain head cover for personal use. These requirements apply to users of half face, full face, filtering face piece (dust masks), and powered air purifying respirators (PAPR). Hours of Performance and Notification: Contractor shall provide testing and training services for employees as requested 24 hours a day, seven days a week, on an as needed basis (excluding Federal Holidays). Whenever possible, requests will be made 24 hours in advance; however, contractor must be able to accept requests for same day service. The VA will reserve a room to accommodate these services. Requests for services will be made either in writing or by telephone by persons designated in writing by the facilities Contracting Officer ™s Technical Representative (COTR). The VA may schedule up to three (3) employees for each session. The Contractor will be provided with the employee ™s name, facility location, appointment time, and room number to report to conduct the services. Each employee shall be trained immediately following their fit testing. The Contractor shall populate VA ™s database at the completion of each testing and training session. The VA shall provide and maintain the database. The contractor shall provide a completed wallet sized card to each fit-tested VA employee. The card shall include the date of fit-testing, the make, model and size of the respirator on which the VA employee was successfully fit-tested. When conditions occur that are neither foreseeable and/or controllable by the contractor (such as severe storms, flooding, hazardous road and travel conditions), delays or exceptions to the required quality of services shall be reported to the COTR. The Contracting Officer or designee will make a determination as to whether such delays in service will be excused.
