- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Description: Solicitation number W91248-10-T-0027A is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-44. The NAICS Code is 621340 and the small business size standard is $7,000,000. This acquisition is being solicited as unrestricted. This requirement is for Sports Medicine Trainer Services. Performance period is for a 6-months base period and three (4) 12-months options. Place of performance is Fort Campbell, Kentucky. Offer Schedule with contract line item numbers, items, quantities, and units of measure, and Performance Work Statement are attached. The provision at 52.212-1 Instructions to Offerors-Commercial Items (Jun 2008) is applicable and is tailored as follows: Paragraphs (b)(5), (d), (e), (h), and (i) are deleted. This provision is also addended to incorporate 52.252-1 Solicitation Provisions Incorporated by Reference (Feb 1998) (Fill-in information http://acqnet.saalt.army.mil/library/default.htm ); 52.204-6 Data Universal Numbering System (DUNS) Number (Apr 2008); 52.204-7 Central Contractor Registration (Apr 2008); 52.216-1 Type of Contract (Apr 1984) (fill-in information, firm fixed price). The provision at 52.212-2(Jan 1999) Evaluation--Commercial Items (Jan 1999) is applicable; evaluation factors are Technical (Certification/License and Experience) and Price. Technical factors are: 1) Provide Certification as a Sports Medicine Trainer by the Kentucky National Athletic Trainers Association Board of Certification and licensed by the Kentucky State Board of Medical Examiners/License of Proposed Trainer; 2). Experience: Provide a written narrative addresing Experience of Proposed Trainer (requires at least one (1) year experience as a sports trainer and employment by or contracted to an accredited medical and/or sports medicine facility)). The Government intends to make a single award to the responsible offeror whose offer conforming to the solicitation will be the lowest price technically acceptable offeror. As part of price evaluation, the Government will evaluate its option to extend services by adding six (6) months of the offeror's final option period price to the offeror's total price. Offerors are required only to price the base and option periods. Offerors shall not submit a price for the potential six month extension of services period. The Government may choose to exercise the Extension of Services at the end of any performance period (base or option periods), utilizing the unit prices of that performance period. See Contract Clauses 52.217-9, Option to Extend Services and 52.217-9, Option to Extend the Term of the Contract. The successful offeror must be registered in CCR (Central Contractor Registration). Offerors are instructed to submit the following documents with offers: Completed Offer Schedule, documentation showing technical capability as stated herein, and Completed copy of FAR Provision 52.212-3, Offeror Representations and CertificationsCommercial Items (Aug 2009), and DFARS Provision 252.212-7000 Offeror Representations and CertificationsCommercial Items (Jun 2005) or a statement that Representations and Certifications are available at the ORCA website. In accordance with FAR Part 9, offeror may be required to provide financial and customer references (name of financial institution where business bank account is located, supplier names where accounts are established, account numbers, names of customers, points of contact, and telephone numbers). Also provide a brief description of previous contracts: services provided, the length of the contract or agreement, and the dollar value, point of contact and telephone number. This information will be used to determine responsibility for the successful offeror.
