- This is a Request for Quote for the replacement of the Dust Collector and Duct work system in the Logistics Support Facility (LSF) at the Mike Monroney Aeronautical Center (MMAC), Oklahoma City, OK, 73169, per the attached Statement of Work (SOW), Drawings, OSHA Safety and Environmental Guidelines, and FAA Clause Sheet. This requirement is 100% set aside for competition among Small Businesses. Your company MUST be registered with a Dun and Bradstreet Number (DUNS) as a small business in the Central Contractor Registry website (www.ccr.gov) for consideration for award. The FAA will not pay for any information received or costs incurred in preparing the response to this Request for Quote. Therefore any cost associated with your submission is solely at the interested vendor's expense. Attached is a copy of the Statement of Work (SOW), Drawings, OSHA Safety and Environmental Guidelines, and FAA Clause Sheet, and a Business Declaration Form. Please respond by reviewing the SOW, Drawings, OSHA Safety and Environmental Guidelines, and FAA Clause Sheet and submitting a filled-out and signed copy of the Declaration form (please submit electronically, email to address below). NOTE TO VENDOR: Terms and conditions which differ, in part or in total, from those contained in the "FAA Clause Sheet" document will not be considered nor accepted by the Government. The terms and conditions herein may only be changed as a result of price negotiations, quantity / payment discounts offered by the Vendor, or changes to delivery terms / schedule for the supplies / services required as agreed to by both parties. In these instances, the Contracting Officer shall clearly document any agreements in the contract for the benefit of both parties prior to contract award. All responses to this market survey must be received by Wednesday, September 22, 2010, 3:00 CST (Oklahoma Time). System discharge shall not return to the building to ensure air quality for Crate Shop employees. ___________The new Dust Collector and Installation for the new duct work will meet OSHA standards.
