Description
This document is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, proposals are being requested, and a written solicitation will not be issued The proposed contract action is a Blanket Purchase Agreement for supplies or services for which the Government intends to solicit and negotiate under FAR 13.303. Interested persons may identify their interest and capability to respond to the requirement or submit offers. This notice of intent is a request for competitive offers, and any quote received prior to Sept 17, 2010 will be considered by the Government. This action is being TOTALLY set aside for SMALL BUSINESSES. NAICS code is 324110. The size standard is 1500 employees. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-44. Only emailed requests received directly from the requester are acceptable. Supplies include items classified under Federal Supply Codes 9130, Liquid propellants - petroleum base; 9135, Liquid propellants - chemical base; and 9140, Fuel oils. The Government intends to award approximately three (3) blanket purchase agreements Companies need to review the following information and must submit the information required in the offer evaluation portion. This information will be reviewed by the Government. Blanket Purchase Agreement Fuel for the Palo Alto Health Care System. Statement of Work 1.Description of Agreement 1.1.The Contractor shall delivery fuels, FOB Destination, in quantities specified by orders under this Blanket Purchase Agreement (BPA) to facilities in the VA Palo Alto Health Care System (VAPAHCS). 1.2.This Blanket Purchase Agreement will expire one year after the effective date. Either party may terminate this agreement by giving thirty (30) days written notice. 1.2.1.After BPA expiration or termination, no additional orders may be placed under this BPA; however, this BPA will remain in effect until all orders placed under this BPA are fulfilled. 2.Specific Tasks-The Contractor Shall 2.1.Provide fuels under federal supply codes 9130, Liquid propellants - petroleum base; 9135, Liquid propellants - chemical base; and 9140, Fuel oils to the Department of Veterans Affairs Palo Alto Health Care System (VAPAHCS) facilities, FOB Destination. 2.1.1.VAPAHCS contains three main facilities; however, other facilities exist throughout the VAPAHCS: Palo Alto Division 3801 Miranda Ave Palo Alto, CA 94304-1290 Livermore Division 4951 Arroyo Road Livermore, CA 94550 Menlo Park Division 795 Willow Road Menlo Park, CA 94025 2.1.1.1.Individual Orders shall contain specific delivery location(s). 2.1.2.Each individual Order shall be independently priced. 2.1.3.Each individual order may add additional, mutually-agreeable terms and conditions, so long they do not conflict with the parent BPA. 2.2.Not accept any instructions issued by any other person(s) than the Contracting Officer. 2.3.Delivery requirements. All deliveries under this blanket purchase agreement (BPA) shall be made within seven (7) days from receipt of order. If the delivery day falls on a holiday, the contractor must then delivery on the next business day. Individual orders may indicate different delivery requirements. 3.Extent of Obligation 3.1.This BPA does not obligate any funds. The Government is only obligated to the extent of authorized purchases actually made under this BPA. 4.Purchase Limitation 4.1.Each individual order under this BPA shall be limited to $100,000.00. 5.Individuals Authorized to Purchase Under This BPA. 5.1.All VA Sierra Pacific Network (VISN 21) Contracting Officers are authorized to purchase under this BPA. 5.2.The VA shall furnish a list of other individuals authorized to purchase under this BPA. 6.Delivery Tickets 6.1.All deliveries under this BPA shall be accompanied by delivery tickets or sales slips that shall contain the following minimum information: 6.1.1.Name of Supplier 6.1.2.BPA number 6.1.3.Date of Purchase 6.1.4.Name of VA Official making purchase 6.1.5.Purchase number 6.1.6.Itemized list of supplies or services furnished 6.1.7.Quantity, unit price, and extension for each item, less applicable discounts 6.1.8.Date of delivery. 6.1.9.Delivery Location 7.Invoices 7.1.An itemized invoice shall be submitted at least monthly or upon expiration of this BPA, whichever occurs first, for all deliveries made during a billing period and for which payment has not been received. These invoices need not be supported by copies of delivery tickets. Solicitation Instructions Offerors shall include in their offer, in the attached spreadsheet, the most-favored customer fuel prices per gallon for the following items for 30 days prior to the release date of this solicitation: "Unleaded Fuel, Octane 87, 89, and 91 "Clear Diesel "Diesel, Red #2 The Government will award BPAs to approximately three offerors with the lowest average prices as indicated by the required information. The government reserves the right to designate a competitive range.
