- THIS NOTICE IS BEING ISSUED TO DETERMINE IF THERE IS SUFFICIENT INTEREST FROM QUALIFIED TOTAL HUBZONE, OR SERVICE DISABLED VETERAN-OWNED SMALL BUSINESSES TO SET THIS PROJECT ASIDE. (Details Below). Prime Contractors who are HUBZone small businesses, 8(a) small businesses, or Service disabled veteran-owned small businesses and who intend to submit a bid for this solicitation MUST submit the following by e-mail to CFLContracts@fhwa.dot.gov or by telefax to 720-963-3360 (Attn: Craig Holsopple) for receipt by close of business (3 p.m. local Denver time) on September 17, 2010: (1) A positive statement of your intention to submit a bid for this solicitation as a Prime Contractor; (2) A copy of the letter from SBA stating date of HUBZone small business certification or acceptance to the 8(a) small business program. In the case of a Service Disabled Veteran Owned Small Business Concern, you must provide proof of eligibility (DD form 214 or a letter of adjudication from the Veterans Administration). This information must be provided in order to determine eligibility - DO NOT SEND COPIES OF YOUR CCR PROFILE; (3) Letter from bonding agent stating your firm's capability to bond for a single project of $8.5 million, and your firm's aggregate bonding capacity; and (4) Provide a list of projects performed as a design/build prime contractor within the last seven years of similar size involving bridge rehabilitation/replacement, retaining wall, geo-technical investigations and recommendations, foundation design, placing embankment, stone masonry wingwalls, public co-ordination activities, minor asphalt paving and permit application. Indicate project specifics and provide references with names and phone numbers. DO NOT SEND INFORMATION ON PROJECTS THAT DO NOT INCLUDE THE ABOVE WORK. The size of a small business firm includes all parents, subsidiaries, affiliates, etc. Refer to the Code of Federal Regulations 13 CFR 121.103 for information on how the SBA determines affiliation and 13 CFR 121.108 regarding the penalties for misrepresentation of size status. All firms should be certified in the Online Representations and Certifications Application (ORCA) located at https://orca.bpn.gov/ A Qualified HUBZone SBC receiving a HUBZone contract for general construction must spend at least 50% of the cost of the contract incurred for personnel on its own employees or employees of other qualified HUBZone SBCs. This requirement may be met by expending at least 50% of the cost of the contract incurred for personnel on its employees or it may subcontract at least 35% of the cost of the contract performance incurred for personnel to one or more qualified HUBZone SBCs. A qualified HUBZone SBC prime contractor may not, however, subcontract more than 50% of the cost of the contract incurred for personnel to non-qualified HUBZone SBCs. PROJECT DETAILS: The Central Federal Lands Highway Division (CFLHD) of the Federal Highway Administration (FHWA) intends to conduct a "two phased" Design-Build (D-B) selection process for the AZ PFH 51-1(2), Control Road (Tonto National Forest Bridge Rehabilitation/ Replacements) Project in Payson, Arizona. The Tonto National Forest Bridge Replacement Project is to design and rehabilitate/construct 7 new bridge replacements for the sub-standard geometric & structural deficient bridges and to hydraulically improve one at-grade stream crossing along Forest Highway 51 (also known as National Forest System Road 64) in Gila County, AZ. The subject bridges and 1 low water crossing on Forest Highway 51 are listed below: •• Webber Creek Bridge, Milepost 5.7, span length = 110 feet Lewis Creek Bridge, Milepost 16.8, s pan length = 30 feet •• Bonita Creek Bridge, Milepost 13.8, Span length - 30 feet •• Ellison Creek Bridge, Milepost 17.4, span length = 45 feet •• Perley Creek Bridge, Milepost 14.7, Span length = 30 feet •• Roberts Draw Bridge, Milepost 19.6, span length = 45 feet •• Moore Creek Bridge, Milepost 15.4, Span length = 30 feet •• Tonto Village Low Water Crossing, Milepost 20.5, span length = 30 feet Work on the 8 structures will involve hydraulic enhancements, roadway geometric improvements and updating Bridge design requirements to the AASHTO HL-93 load carrying criteria. The existing seven bridges are either single or double span and had their wooden deck replaced with a concrete slab in the late 1950's. Most of these bridges are historic in origin having been built by the Civilian Conservation Corps (CCC) in the 1930's. All proposed improvement on the Control Road project will need to follow guidelines & mitigation measures contained in the National Environmental Policy Act (NEPA) document for Arizona Forest Highway 51-1(2), Control Road, which is anticipated to be completed in December 2010. Roadway/bridge quantities for the Design/Build project are expected to involve improvements to roadway approach's at all 7 bridge structures, bridge/drainage structure construction, guardrail replacement, retaining wall design, geo-technical investigations and recommendations, foundation design, placing embankment, stone masonry wingwalls, public co-ordination activities, minor asphalt paving and permit application. Interested Design-Build firms/teams are requested to submit a technical package demonstrating their experience with design/build bridge projects similar in scope and value to the subject project. Evaluations will be based upon the firms specific and verifiable experience on Transportation Design/Build projects specializing in new bridge construction. A shortlist of three or more Design-Build teams will be invited to participate in Phase II - Request for Proposals (RFP), which is anticipated to be released in mid to late January, 2011. Estimated cost for the project is $3.5 to $8.5 million.
