- STATEMENT OF WORK (SOW) DEPARTMENT OF VETERANS AFFAIRS - WILMINGTON VA MEDICAL CENTER SUPPLIES OR SERVICES GENERAL DESCRIPTION: 1. Contractor shall provide Food and Drug Administration (FDA) approved Mammogram, Ultrasound, MRI/MRA Services to all referred patients of the Department of Veterans Affairs, Wilmington VA Medical Center, Wilmington De. Services shall be performed at contractor's facility. The services Shall be provided on a scheduled or emergent basis for VAMC's use in determining diagnosis and patient treatment management plans. Contractor to provide services to include: space, equipment, certified personnel, incidentals and results/reports. Contractor's equipment must have appropriate certification and have routine maintenance and quality assurance reviews. Evidence of this must be included in the Contractor's proposal. Contractor to send all invoices directly to the fee basis department at the Wilmington VA Medical Center at the following address: 1601 Kirkwood Hw, Wilmington DE 19805, ATTN: Fee basis department. 2. DESCRIPTION OF FACILITIES: Contractor's facilities shall be accessible for wheelchair and stretcher, as well as ambulatory patients. The contractor shall take full responsibility for the patient's well-being and safety once the patient enters its facility. Contractor shall have the ability to appropriately monitor inpatients with IVs, Oxygen and provide emergency care as needed, i.e.; resuscitation, CPR, life saving. 3. CONTRACTORS PERSONNEL: Technical Staff. All Diagnostic technologists shall be certified by the American Registry of Radiologic Technologists (ARRT). All Radiologists are to be credentialed and submit evidence of peer review on VAMC requested procedures. 4. STAFF/SCHEDULING PROCEDURES Wilmington VAMC clerical staff shall fax physician's orders to contractor's office. The Contractor's staff shall be responsible for scheduling exams ordered on VAMC patients and contacting them to notify him/her of the scheduled time, provide directions to the Contractor's facility, and provide any special instructions relative to the examination. Notification of failure to reach patient, canceled and missed appointments will be faxed to the Nuclear Medicine clerical staff. Callbacks. The contractor shall contact the patient - for any repeat exams or specialized views deemed necessary at no additional cost. 5. TIMELINESS: Contractor shall provide appointments within three business days of requested date. Emergent studies may be needed within 24 hours of physician's order. A CD/DVS with the complete exam shall be delivered to VAMC Radiology Service within 24 hours after exam has been completed. 6. REPORT AND IMAGE DELIVERY: Provide individual patient reports including data: scan type, referring physician, scan time, administration of contrast, dose, etc. The reports shall be typed by the Contractor's transcription team and faxed to the Wilmington VAMC Nuclear Medicine department within 48 hours of completion of exam. The Wilmington VAMC Nuclear Medicine staff will scan the completed report into the patient's electronic file. If the images/exam is unreadable or of poor quality, then the Contractor shall be notified and required to perform another study on the patient free of charge within three (3) business days after notification from the VA. 7. QUALITY ASSURANCE AND QUALITY CONTROL: Contractor shall have a Quality Assurance program, for equipment and personnel. Copies of files or logs shall be made available on request. Proficiency reports are received from the contractor on a quarterly basis and reviewed by radiologist at the Wilmington VAMC. Contact Irene J. Mancuso with any questions at 302-994-2511 x7525.
