- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotes are being requested and a written solicitation will not be issued. The solicitation number, VA-69D-10-RQ-0647, is issued as a request for quotation (RFQ). This document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-38 effective February 1, 2010 and Veterans Affairs Acquisition Regulation Supplement (VAAR) current to January 2010. The North American Industry Classification System (NAICS) Code for this acquisition is 334513, Size Standard: 500 Employees. This procurement is unrestricted. Offeror's quotes are hereby being requested and shall be submitted on offeror's letterhead or bid form and shall contain the name, title, and signature of person authorized to submit quotes on behalf of the contractor. Offeror shall provide complete system. Offeror shall provide the following Brand Name Only Lumenis Items: Description: Item 1: Lumenis Selectra II Glaucoma Laser System, - 1 EACH Price $___________ Discount (if applicable): $___________ TOTAL PRICE FOR ALL ITEMS $____________; Total Price shall include all equipment delivery charges and with 2 Operator manuals and 2 Service manuals. The only technically acceptable items are the Brand Name Only Complete Lumenis Selectra II Glaucoma Laser System Salient Characteristics: 1) Use a Q-switched, frequency-doubled Nd:YAG; 532 nm wavelength green laser. 2) Selective Laser Trabeculoplasty (SLT) laser treatment to reduce IOP without coagulating the Trabecular Meshwork. 3) clip on laser. 4) Portable. 5) 120 - 230 VAC, 50/60 Hz, switchable input. 6) Air convection cooling system. 7) 532nm Wavelenght. 8) Dimensions: Laser head: 5.4" x 7" x 11.4", Power Supply: 17" x 12" x 8.5". 9) Single pulse-on demand burst mode with 3 nanosecond pulse duration. 10) Q-Switched Frequency Doubled Nd:YAG laser source. Site Visit: There will not be a Site Visit Delivery shall be FOB DESTINATION to the Edward Hines VA Medical Center, Warehouse Receiving/Shipping, 5000 S. 5th AVE, Hines, IL 60141; Attention: Sharisha Chunchu. The Contractor is responsible for all freight charges. Offer must include product literature, product samples (if requested), technical features, warranty provisions, and any training and or installation that will be required. The provision at 52.212-1, Instructions to Offeror--Commercial Items, applies to this solicitation. Award will be made to the offeror representing the LOWEST PRICE TECHNICALLY ACCEPTABLE (exactly the brand name only items required). All quotes will be evaluated in accordance with technical capability, compliance with the solicitation requirements and price. Price is important as the Government intends to award to the low priced offeror offering the required items. The government intends to evaluate proposals and award without discussions with offerors. Therefore, the offeror's initial proposal should contain the offeror's best terms from a cost or price and technical standpoint. The Government reserves the right to conduct discussions if the Contracting Officer later determines them to be necessary. The provision at 52.212-3, Offeror Representations and Certification--Commercial Items, applies to this solicitation.
