- SS Letter Statement of Work This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is F3Z3680176A001; this solicitation is a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-44 effective 08 Jul 2010; Defense DCN 20100820 effective 20 Aug 2010, and AFAC 2010-0402 effective 02 Apr 2010. This acquisition is unrestricted; The North American Industry Classification System (NAICS) code is 541511. The business size standard is $25,000,000. The Federal Supply Class (FSC) is D399. The Standard Industrial Classification (SIC) is 7371. Travis AFB has a requirement for a software support. This requirement is sole sourced to: Brittingham Software Design Inc, CAGE: 1MX22. Health and Wellness Center is outfitted with the software provided by BDSI. 0001 - Motivation Base System - 5 Each 0002 - Motivation Member services Module - 5 Each 0003 - Motivation Check-In Module - 5 Each 0004 - Motivation Annual Site Management Fee for single location - 1 Each 0005 - Customization and Training, convert V11 to Motivaton V12 - 10 Each Software upgrade will be in accordance with the attached Statement of Work Period of Performance 01 Oct 2010 - 30 Sep 2011. Quotes must be valid until 02 October 2010. Award shall be made in the aggregate, all or none. The following provisions and/or clauses apply to this acquisition. Offerors must comply with all instructions contained in: FAR 52.232-18 Availibility of Funds. - Funds are not presently available for this contract. The Government's obligation under this contract is contingent upon the availability of appropriated funds from which payment for contract purposes can be made. No legal liability on the part of the Government for any payment may arise until funds are made available to the Contracting Officer for this contract and until the Contractor receives notice of such availability, to be confirmed in writing by the Contracting Officer.
