- Solicitation for Fitness Assessment Cell This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation: proposals are being requested and a written solicitation will not be issued. Solicitation number F3S40237A001 is hereby issued as a request for quote (RFQ). The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-12, 4 August 2006. DESCRIPTION OF REQUIREMENT: CLIN 0001: Quantity 12 months The contractor requires 2 Fitness Assessment Cell Staff Members services for the 354 Force Support Squadron (FSS) Eielson AFB, AK. Contractor Personnel will perform duties at the Fitness Center according to the requirements contained in the Performance Work Statement (PWS) and in accordance with AFI 36-2905, effective 1 July 2010. The contractor shall perform services in accordance with ethical, professional, and technical standards. The contractor shall provide all management, tools, supplies, and labor to perfom as contracted qualifed Fitness Assessment Cell Staff Members (referred to as "contractor" herein) for the 354 FSS Eielson AFB, Alaska, in accordance with the requirements contained in the PWS, and the professional standards of the Joint Commission (JC) and the Air Forces Inspection Agency's Health Services Inspection. See attached Performance Work Statement. Period of Performance covering 1 October 2010 - 30 September 2011. (End description).
