Solicitation Number: VA-241-10-RQ-0610 Notice Type: Combined Synopsis/Solicitation Synopsis: This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in subpart 12.6, in conjunction with the policies and procedures for solicitation, evaluation, and award as prescribed under FAR 13.1, and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. All offerors MUST be registered on www.ccr.gov, MUST provide a Valid DUNS Number and TAX ID Number, along with a detailed pricing and delivery quote. If any of the items are not sent along with the quote, the response may be considered non-responsive. (ii) The solicitation number is VA-241-10-RQ-0610 and is issued as a Request For Quotation (RFQ) (iii) The provisions and clauses incorporated into this solicitation document are those in effect through Federal Acquisition Circular 2005-35, July 14, 2009. Provisions and clauses incorporated by reference have the same force and effect as if they were given in full text. The full text of the Federal Acquisition Regulations (FAR) and Veterans Affairs Acquisition Regulations supplement (VAAR) can be accessed on the Internet at https://www.acquisition.gov/far/ (FAR) and http://vaww.appc1.va.gov/oamm/vaar (VAAR) (iv) This solicitation is issued as full and open competition. The North American Industry Classification System (NAICS) code is 423450, and the size standard is 100 employees. (v) This requirement consists of three (3) line items: 1.Intego PET Infusion System - qty (1) 2.Installation of Intego PET Infusion System - qty (1) 3.Vial, Shield, Intego - qty (4) (vi) The following STATEMENT OF WORK is a detailed description of the requirements for this solicitation: Basic SOW for the PET Infusion Pump: For infusion of Sodium Fluoride (F18-NaF)(radio-pharmaceutical) administration in Positron Emission Tomography (PET) imaging in the Nuclear Medicine Department. An powered injector reduce the time and handling of this radioactive pharmaceutical. Technologists are able to stay a distance from the patient while the radiopharm is being infused. This reduces radiation exposure by 20%. Ability to prepare patient-specific doses on demand with weight based dosing. FDG Delivery System - MUST HAVE THE FOLLOWING TECHNICAL SPECIFICATIONS: Weight:Approximately 730 lbs (331 kg) Dimensions:Approximately 24 inches (60 cm) wide, 34 inches (86 cm) deep, 53 Inches (136 cm) high Maximum radioactivity: 750 mCi (27.7 GBq) for shielding effectiveness 700 mCi (25.9 GBq) for dose preparation Maximum FDG volume: 30 mL Maximum FDG activity concentration:100 mCi/mL (3,700 MBq/mL) Dose range:1 - 25 mCi (37 - 925 MBq) FDG dose accuracy:+/- 2% of the measured dose, excluding the ionization chamber calibration Flow rate:1 mL/sec System radiation profile:< 2 mrem/hr (0.02 mSv/hr) at 12 inches (30.5 cm) from the surface of the cart at maximum activity (vii) Delivery FOB Destination to VA Medical Center, 1400 VFW Parkway, West Roxbury, MA 02132-4927. More information will be provided upon contract award. (viii) 52.212-1 Instructions to Offerors - commercial Items, apply to this solicitation with the following addenda, (ix) 52.212-2, Evaluation - Commercial Items: FAR provision 52.212-2 applies to this solicitation. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (i)Technical capability of the item offered to meet the Government requirement (ii)Price (iii)Past Performance All evaluation factors other than cost or price, when combined, are approximately equal to price. (x) 52.212-3 Offeror Representations and Certifications - Commercial Items- Offeror's are to include a COMPLETED COPY OF PROVISION 52.212-3 WITH THE SUBMITTED QUOTE, unless offeror has a current ORCA certification.
