Description
RECOVERY - This procurement will be firm-fixed price and is funded in whole by the American Recovery and Reinvestment Act (ARRA) of 2009 and is subject to reporting requirements of the Act. TAS No. 14902101. Project No.D34N23. RECOVERY: THIS IS AN AMERICAN RECOVERY AND REINVESTMENT ACT (ARRA) PROCUREMENT: In compliance with the transparency and accountability requirements associated with the supplemental appropriations provided by the American Recovery and Re-Investment Act of 2009, Pub. Law 111-5, The Bureau of Indian Education, NMNS -T'iists'oozi' Bi'o'lta (Crownpoint Community School), Crownpoint, NM, is soliciting quotes for consultant services. *******************Special Recovery Reporting Requirements********************** The Contractor shall register and report all project information as listed in FAR PART 52.204-11, using the online reporting tool available at www.federalreporting.gov. Cumulative reporting to www.federalreporting.gov will begin with the October 10th reporting deadline. Contractors must check this online site often for updates and monitor finalization of the interim rule published in the Federal Register at 74 FR 14639. Failure on the part of the Contractor to provide, maintain, and report the required reporting and special reporting information as required by FAR 52.204-11 to BIA Contracting Officer within specified days following award AND/OR report information to www.federalreporting.gov shall constitute a material breach of the contract upon which BIA may immediately terminate a resultant contract for cause. A fixed price contract with estimated quantities will be awarded. Receipt of award is contingent on contractor meeting the requirements of ARRA reporting. Contractor must be registered with both the Central Contractor Registration (CCR) at ccr.gov and Federal Reporting at www.federalreporting.gov websites in order to receive any ARRA funded award from the Federal Government. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation, RA000910383 is issued as a Request for Quotes (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-43. All businesses shall provide a breakdown of their costs to demonstrate that at least 51% of the cost of contract performance incurred for travel, supplies, training materials, etc. shall be performed and incurred by the prime. All responsible business sources may submit a quote, which will be considered by the Government. Please provide a breakdown of your costs. The breakdown shall include a daily rate and an estimate for your travel costs which shall be reimbursed at the actual travel costs in accordance with the Federal Travel Regulations. The Period of Performance (POP) shall be from date of award to June 30, 2011. All work shall be performed in accordance with the Statement of Work. The contract shall be awarded as a firm fixed price contract for one school year and four option years. This office is requesting quotes on the following item: Provide all travel, supplies, materials and personnel to provide Reading Testing Services for Bureau of Indian Education School, located on the Navajo Indian Reservation in Crownpoint, New Mexico in accordance with the Statement of Work (SOW). The school is: BIE, NMNC - T'iists'oozi' Bi'o'lta (Crownpoint Community School) in Crownpoint, New Mexico. Statement of Work, Consultant Services for Reading Crownpoint Community School SCOPE: On-site school consultation visits are needed at T'iists'oozi' Bi'o'lta (Crownpoint Community School) in September, October, November, December, January, February, March, April, May, June during the 2010/2011 School Year. BACKGROUND: T'iists'oozi' Bi'o'lta (Crownpoint Community School) is a restructuring school in the 'Support our Schools' program. As such, the school is focused on improving student achievement in reading. An external technical assistance provider with a proven program in increasing student achievement is needed to guide the improvement process. The school wishes to refine the implementation of the writing lesson plans program in grades K-8th and expand the writing lesson plan program to grades K-8th, Scholastic Reading Inventory & NWEA (what type programs). OBJECTIVES: The Contractor will assist the school with the school by working with teachers in grades K-8th to raise the level of student achievement, to ensure adequately yearly progress for all students and to train academic staff in strategies to assure student success in future years. This reading program is with emphasis on improving reading skills in all grades levels using all components provided by the reading program. Formative assessments used to determine reading growth is Scholastic Reading Inventory (SRI) and Northwest Evaluation Assessment (NWEA). SRI is administered every eight weeks and NWEA is administered three times in a school year. The Contractor will provide a total of 14 (fourteen) on-site visits by 1 (one) facilitator. There will be 2 (two) visits per month starting in September through June. Deliver all material: Formative tests for reading grades K-8th. The formative tests include reading questions for each Performance Objective (PO) tested; Proof of Mastery Grade Sheets. Proof of Mastery Grade Sheets document the grades students get on each contract; and Meet with administrators to review the beginning of school procedure which include: blocking-time analysis, curriculum meetings, lesson plan templates, and proof of mastery grade sheets. Meet with teachers to review the following: Lesson plan templates; Review pacing calendar and emphasize accountability; Mastery grade sheets; Identify students not achieving mastery; Plan remediation for each student not achieving mastery; and Report findings and recommendations. Review and introduce test taking skills and procedures. Review previous months and give and explain procedures for giving the mock tests. Review data from the first mock test with teachers and administrators and plan remediation. A copy of the report will be sent to the Education Line Office. Teachers and facilitator will plan remediation. Review previous month and review student remediation plans and respond to intervention. Review response to intervention and prepare for the second mock test. Review mock tests data to prepare for intense remediation and response to intervention in preparation for the next tests. Review test results with administrators. Provide pacing calendars in reading, lesson plan formats that mirror the pacing calendar, grade sheets that mirror the pacing calendar in reading, response to intervention/remediation plans that mirror the pacing calendar and formative tests that mirror the pacing calendar. Provide all tests and scoring of all the mock tests. Written summary report of the professional development activities. Delivery: On-site visits from date of award to June 30, 2011. Government Furnished Property: Meeting rooms, student data for reading and CORE reading materials at the T'iists'oozi' Bi'o'lta (Crownpoint Community School). Security: Contractor will sign in as a school visitor and obtain a visitor pass each day they are at the T'iists'oozi' Bi'o'lta (Crownpoint Community School). A background check is required, names of facilitators will be required. The onsite visits will be performed on the campus of the T'iists'oozi' Bi'o'lta (Crownpoint Community School). All written reports and scoring of mock test may be completed off-site. Period of Performance is from the date of award to June 30, 2011. (one base year with 4 option years) RESPONSIBILITIES OF ALL SERVICE PROVIDERS: The service provider is to report to a designated time and place for orientation to the organizational structure for service provision and the procedures delineated for providing the above specified services. QUALIFICATIONS OF SERVICE PROVIDERS: 1. Bachelor 's Degree plus 30 graduate hours; 2. Two years experience in a school and Licensures in Reading. The contractor shall have a favorable background determination issued by the BIE, Security Office. Please include your TAX ID Number, CAGE Number (obtain at www.ccr.gov), and DUNS Number when submitting your quote. The offeror shall have a favorable screening (applicant and suitability) determination must be made by the Bureau of Indian Affairs (BIA), Personnel Security and Suitability Program in accordance with the Background Investigations Clause. The Offeror shall provide the lump sum cost with a breakdown of their cost to provide this service and respond to the Evaluation Factors delineated at the end of this synopsis. BACKGROUND INVESTIGATION: In accordance with 25 U.S.C. 3201 (P.L. 101-630), Indian Child Protection and Family Violence Prevention Act; 42 U.S.C. 13041, Crime Control Act of 1990; 441 Departmental Manual, Personnel Security and Suitability Requirements all positions with duties and responsibilities involving regular contact with or control over Indian children are subject to a background investigation and determination of suitability for employment. Accordingly, all such positions require a favorable screening (applicant and suitability) determination made by the Bureau of Indian Affairs (BIA) Office of Indian Education Programs (OIEP), Personnel Security and Suitability Program; and a favorable background investigation determination conducted by the Office of Personnel Management (OPM). A favorable screening (applicant and suitability) determination must be made by the Bureau of Indian Affairs (BIA) Office of the Indian Education Programs (OIEP), Personnel Security and Suitability Program prior to contract award or made within 60 days of incorporation of this clause into a current/existing contract. Pending completion of a background investigation by OPM, the Contractor who has been granted a favorable screening (applicant and suitability) determination must be within the sight and under the supervision of a Government employee who has had a completed background investigation (42 U.S.C 13041 (3)) during contract performance until a final background investigation determination has been made. Prior employment with the Federal Government, including BIA, OIEP or the Bureau of Indian Affairs, does not assure that the appropriate screening or investigation was conducted nor does it alleviate or guarantee a favorable screening (applicant and suitability) determination or background investigation. Prior to an award or during the 60 day period discussed in paragraph two, it is solely the company, individual or contractor's responsibility to contact the BIA, OIEP Personnel Security and Suitability Program at 505-563-5287/5288/5289/5294 in order to obtain a complete security package, which includes but is not limited to appropriate investigative forms and/or inquire about their previous investigation; and, obtain a screening (applicant and suitability) determination, and subsequent final background investigation. If an unfavorable screening (applicant and suitability) determination is made, a purchase order shall not be awarded to the individual who received an unfavorable screening or to the company who has proposed to employ the individual to perform the work. The individual and or the company who proposed to employ this individual may be determined to be non-responsible if an unfavorable screening (applicant and suitability) determinations is made, which will make them ineligible for purchase order award. If an unfavorable final background investigation determination is made, the purchase order may be terminated and the employee in question shall not be authorized to provide service to the BIA, OIEP, if the services involve contact with or regular control over Indian children. REQUIRED QUALIFICATIONS OF SERVICE PROVIDER: Award will be based on the Best Value to the Government, FAR 52.212-2, Evaluation. The Evaluation Factors are in descending order of importance. All evaluation factors other than cost or price, when combined are significantly more important than cost or price. The Evaluation Factors are: 1) Technical Approach: Describe your technical approach to accomplish each of the objectives and/or tasks delineated in the Statement of Work; 2) Past Experience: List contracts that are of comparable size, complexity and similar; 3) Past Performance: Provide recent and relevant contracts for the same or similar items, contract numbers, points of contacts with telephone numbers and other relevant information; 4) Knowledge and familiarity of the school and location; and 5) Price/Cost. Due to the urgency of the requirement, the response time for quotes due, is short.
