- CHIMNEY STACK REPAIR (608-08-101) BACKGROUND The boiler plant chimney is spalling and debris is falling from the chimney. There is damage from weather and water both internally and externally. STATEMENT OF WORK Provide services to accomplish the following tasks, assuring compliance with all OSHA and VA guidelines for lock out / tag out, fall protection and confined space entry: TASK 1: Interior Work Provide all labor, equipment and material, except utilities, to complete the following: "Remove all existing delaminated shotcrete around perimeters of areas already missing shotcrete, "Prepare substrate using a high-pressure washer and potable water "Apply one (1) coat of Henkel HES cement per manufacturer's recommendations at an average thickness of 1" (approximately 200 square feet). "Clean up and dispose of spent debris in accordance with federal, state and local regulations. Task 2: Exterior work: Provide all labor, equipment and material, except utilities, to complete the following: "On a spot point basis, cut to 3/8" depth or to sound mortar all deteriorated brick mortar joints and repoint with mortar matching existing as closely as possible (approximately 1,754 square feet). "Remove and replace all badly spalled, cracked, or deteriorated brick with new brick to match existing as closely as possible (approximately 500 pieces). "Restoration cleans exterior masonry using a combination of restoration detergents and pressure washers (approximately 3,128 square feet). "Apply a wet-on-wet saturation of Siloxane damp proofing (approximately 3,128 square feet). Window glass and metal will be covered prior to damp proofing. "Clean up and dispose of spent debris. OTHER CONSIDERATIONS FOR PROPOSAL 1.Perform work in compliance with Specifications: 01 00 00 - General Requirements 04 05 31 - Masonry Tuck Pointing 04 05 13 - Masonry Mortaring 2.The Contractor shall arrange for all waste to be recycled or disposed of in accordance with all Federal, State, or local laws and regulations. 3.Bidders must submit a copy of the following as part of their bid: "Safety Plan. "Respiratory Protection Program (or, if a subcontractor is removing the existing insulation containing mold, the subcontractor's Respiratory Protection Program). 4.Additionally, the following must be submitted and approved by the VA before the contractor begins work on site: "Activity Hazard Analysis (AHA). A blank AHA form is available at http://www.swf.usace.army.mil/pubdata/safety/PDF/blank_AHA_Form.pdf. "Schedule. "Material Safety Data Sheets (MSDS) for any hazardous materials to be brought on site. 5.The provisions of all applicable NFPA, OSHA, EPA, and VA codes, Federal, State, and local regulations shall be followed throughout the course of this project. Applicable VA publications and design manuals are available to the successful bidder at www.va.gov/facmgt. 6.The provisions of Specification 01 00 00 (see Attachment A), General Requirements, shall be followed at all times.
