Description
Performance-based Work Statement: Attachment to RFQ No. FA5205-10-Q-KN03 Request For Quotation: Asbestos Training at Misawa Air Base, Japan (i) This is combined synopsis/solicitation for commercial items, prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The information we request below is the minimum information required for us to make an award. If the applicable information is not provided entirely, your quotation will not be considered for award. (ii) This solicitation is being issued as a Request for Quote (RFQ) No. FA5205-10-Q-KN03. Please submit a written quotation NLT 2 Aug 10, 14:00 (Japan Standard Time) by Email. POC: Mr. Kazuhiro Nakamura, Email: kazuhiro.nakamura.jp@misawa.af.mil, Tel: 011-81-176-77-3496. (iii) This solicitation document and provisions/clauses are those in effect through Federal Acquisition Circular (FAC) 2005-42 (as of 26 Jul 10). (iv) The NAICS code 541620 applies to this solicitation. (v) Quotation is requested for on-site asbestos training. 1) 5 day asbestos initial supervisor in English up to 9 trainees; 2) 3 day asbestos initial designer in English up to 2 trainees; 3) 4 day asbestos initial worker in English up to 10 trainees; 4) 4 day asbestos initial worker in Japanese up to 3 trainees; 5) 1 day asbestos refresher worker in Japanese up to 22 trainees; 6) 1 day asbestos refresher supervisor in English up to 17 trainees; 7) a half day asbestos refresher inspector in English up to 8 trainees; 8) a half day asbestos refresher planner in English up to 2 trainees; and 9) 1 day asbestos refresher designer in English up to 10 trainees. (vi) Provide on-site asbestos trainings and English and Japanese training materials to each trainee at Misawa Air Base, Japan. The contractor shall issue certificates of each provided initial training and renew certificates of refresher trainings accredited by the state of Hawaii and Pennsylvania IAW U.S. Environmental Protection Agency (EPA) and the Occupational Safety and Health Administration (OSHA). The contractor shall have an EPA and /or a state accredited training certificate. (vii) Start on-site asbestos trainings by the end of 30 Sep 2010 at Misawa Air Base, Japan, and complete all the trainings within 20 calendar days (CD) by the end of Oct 2010, excluding Saturdays, Sundays and the U.S. legal holidays IAW the Performance-based Work Statement (PWS). The contractor will be notified of the actual training dates and numbers of trainees for each training at least 20 calendar days before the first training date. (viii) The provision at FAR 52.212-1, Instructions to Offerors' Commercial Items, applies to this acquisition. (ix) FAR 52.212-2, Evaluation - Commercial Items (Jan 1999), (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (i) Technical Capability: (1) Certificate of EPA and/or a state approved asbestos training provider for asbestos training courses listed in para. 1.3 of the PWS; and (2) Certificate of EPA and/or a state approved training instructor. (ii) Price: Offerors shall submit copies or supporting documentation of each certificate to the Government at the time of offer submission. Technical proposal will be evaluated to determine technical capability on a pass/fail basis, and the technically unacceptable offer will not be considered for award. Price of the technically acceptable offers will be evaluated. (b) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. (x) The provision at FAR 52.212-3, Offeror Represenations and Certifications-Commercial Items, applies to this acquisition. (xi) FAR 52.212-4, Contract Terms and Conditions- Commercial Items applies to this solicitation. (xii) FAR 52.212-5, Contract Terms and Conditions Required to Implement Statues or Executive Orders- Commercial Items, applies to this solicitation. Specifically, the following clauses cited are applicable to this solicitation, FAR 52.222-3, Convict Labor, FAR 52.222-21, Prohibition of Segregated Facilities, FAR 52.222-26, Equal Opportunity, FAR 52.222-36, Affirmative Action for Workers with Disabilities, FAR 52.225-13, Restrictions on Certain Foreign Purchase, FAR 52.228-3, Workers' Compensation Insurance (Defense Base Act), FAR 52.228-4, Workers' Compensation and War-Hazard Insurance Overseas, and FAR 52.232-33, Payment by Electronic Funds Transfer -Central Contractor Registration. (Xiii) The above Provisions and Clauses may be obtained via the Internet at http://farsite.hill.af.mil.
