This is a combined synopsis/solicitation, based on a Brand Name Or Equal basis for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes only the solicitation; proposals are being requested and a separate written solicitation will not be issued. This procurement is not a small business set-aside; any responsible source may submit an offer. This combined synopsis/solicitation applies and is issued as a Request for Quotation. AGENCY'S NEED:Rehab(Rehabilitation) Trailer Kit.ITEM NO. 0001: Rehab Trailer Kit For Rescue, QTY: 1 ea,Mfr Name: CREW BOSSMfr P/N: Crew Boss Rehab Trailer Kit CBRTRL or equal. Rehab Trailer Kit must have the following minimum salient chracteristics: Trailer Physical Specifications must meet the following: (1) outside Dimensions (excluding tongue): 7.5' W x 8' L x 8.5' H (2) outside Dimensions (with tongue): 7.5' W x 11' L x 8.5' H (3) packed weight capacity: 2999 lbs. Trailer must meet a comprehensive mobile facility with the equipment and supplies necessary to set up an effective on-scene rehab site, based on NFPA 1584 requirements The multi-purpose 6' x 8' trailer must be able to be easily transport behind almost any vehicle with tow hitch capability. Rehab shelter must meet the following dimensions 16' x 16'. Must be able to provide 205 sq.ft. in an enclosed octagonal shelter shape, has removable wall panels, windows, 2 doors, roof vent, whiteboard kit, and must have the ability to handle a variety of options for climate control. Rehab trailer must be able to support a climate-controlled EMT treatment facility and must have a large 9' deployable fold-out awning for additional shelter outside and ground tarp system that can provide an additional 100 square feet of clean, protected space for environmental relief. Must include collapsible seating, toilet with a separate shelter, medical supplies organizer with writing desk, hand washing station, coolers, misting fan, extra tarps. These requirements are to support basic and advanced medical treatment and care. The rehab trailer must have an environmentally-controlled interior with a removable 8' storage bench with seat cushions that serves multiple use. Trailer must be able to clear quickly of equipment, must have a rear gate to create a full-length gap-free ramp able to roll out equipment kart and bench. Rehab trailer must have air conditioning and heat for climate control for rehab of firefighters. Multi-functional exhaust fan for circulation and climate control. Ceiling utility hangers (for IV bags, air masks, etc) and multiple wall hooks to support fire and medical equipment. Must have privacy curtains to support medical treatment and decon patients during disrobing to check for injuries and change out of contaminated clothing in a secured and closed environment. Rehab trailer must have finished and completely washable floor and interior surfaces for decontamination purposes to meet NFPA 1500 requirements. Electric Misting Fan must meet the requirement for low ambient temperature as much as 30, cool draft Blue 18" High-Volume misting fan which must be an industrial grade misting fan made for efficient, effective cooling under the most extreme conditions. Requirements as follows: must be able to effectively reduces body temperatures to combat heat stress, efficiently lowers air temperatures as much as 30 in areas up to 1000 sq. ft., extremely portable self-contained design, 18" Industrial Grade high volume Fan with sealed motor and entire system must be designed to stand up in the most extreme environments. Must be durable, dependable, designed to last in tough environments. Must be designed with materials of polymers, powder-coated metals, and corrosion-resistant hardware throughout. Electrical system requirements as follows: 110 volt System, minimum inverter requirement: 400/750 watts, GFI Plug, 25' power cord, multi function toggle switch, self priming pump with advanced filter design, and the dimensions (fan in operating position): 54" H x 21" W x 17" D, and weight (empty tank): 32lbs. Rehab trailer must be able to meet command and control requirements for incident command methology, must be portable and reusable. Must be supplied with portable white board kit which include the following: (4 ea) broad and (4 ea) fine tip dry erase markers, (1) eraser, and a durable, (1 ea) waterproof 4" x 29" storage tube with a carry strap. (35 ea) flexible white plastic static cling dry erase sheets each measure 27" x 34". Rehab trailer must be able to support a large insulated water cooler (5 gallon capacity) with cup dispenser. Must have extra storage space for food and water supplies, hot mad cold water washing station able to hold 6 gallons of water for washing faces and hands as needed. Unit power source must be able to plug into 12Volt electrical outlet and be supported by hooks on the outside of the trailer designed to hold it. The salient physical, functional, or performance characteristics that "equal" products must meet are specified in the solicitation.(b) To be considered for award, offers of "equal" products, including "equal" products of the brand name manufacturer, must- (1) Meet the salient physical, functional, or performance characteristic specified in this solicitation;(2) Clearly identify the item by- (i) Brand name, if any; and (ii) Make or model number; (3) Include descriptive literature such as illustrations, drawings, or a clear reference to previously furnished descriptive data or information available to the Contracting Officer; and(4) Clearly describe any modifications the offeror plans to make in a product to make it conform to the solicitation requirements. (d) Unless the offeror clearly indicates in its offer that the product being offered is an "equal" product, the offeror shall provide the brand name product referenced in the solicitation.(End of provision) 52.212-3 Offeror Representations and Certifications-Commercial Items. (AUG 2009) An offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically at http://orca.bpn.gov. If an offeror has not completed the annual representations and certifications electronically at the ORCA website, the offeror shall complete only paragraphs (c) through (m) of this provision. (a) Definitions. As used in this provision- "Emerging small business" means a small business concern whose size is no greater than 50 percent of the numerical size standard for the NAICS code designated. "Forced or indentured child labor" means all work or service- (1) Exacted from any person under the age of 18 under the menace of any penalty for its nonperformance and for which the worker does not offer himself voluntarily; or (2) Performed by any person under the age of 18 pursuant to a contract the enforcement of which can be accomplished by process or penalties. "Place of manufacture" means the place where an end product is assembled out of components, or otherwise made or processed from raw materials into the finished product that is to be provided to the Government. If a product is disassembled and reassembled, the place of reassembly is not the place of manufacture. "Restricted business operations" means business operations in Sudan that include power production activities, mineral extraction activities, oil-related activities, or the production of military equipment, as those terms are defined in the Sudan Accountability and Divestment Act of 2007 (Pub. L. 110-174). Restricted business operations do not include business operations that the person (as that term is defined in Section 2 of the Sudan Accountability and Divestment Act of 2007) conducting the business can demonstrate- (1) Are conducted under contract directly and exclusively with the regional government of southern Sudan; (2) Are conducted pursuant to specific authorization from the Office of Foreign Assets Control in the Department of the Treasury, or are expressly exempted under Federal law from the requirement to be conducted under such authorization; (3) Consist of providing goods or services to marginalized populations of Sudan; (4) Consist of providing goods or services to an internationally recognized peacekeeping force or humanitarian organization; (5) Consist of providing goods or services that are used only to promote health or education; or (6) Have been voluntarily suspended.
