Description
This is a combined synposis/solicitation to conmercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is AG-7604-S-10-0040 (PR 417240). The solicitation is a request for proposed (RFP) for a Firm Fixed Price award. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition circular 2005-38. This solicitation is a Small Business Set Aside. NAICS Code 334290, small business size standard 1,500 employees. See below for a list of equipment/supplies, contract line item number(s) and items, quantities and units of measure FOB destination: Delivery Location: Monongahela National Forest, Radio Replacement, ATTN: Bruce Cross or Gary Procter, 3213 Cove road, Roanoke, VA 24017. The vendor must be capable of procuring and delivering the equipment listed within the contract within 30 days of contract award. The provision at 52.212-1, Instructions to Offerors--Commercial, applies to this acquisition. The provision of 52.212-2, Evaluation--Commercial items, if used, and the specific evaluation criteria to be included in paragraph (a) of that provision is: Evaluation factors and significant subfactors as stated in the solicitation are as follows: Award will be made on the basis of the lowest evaluated price of a proposal meeting or exceeding the acceptability standards for non-cost factors. (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers. The non-cost factors are: (l) Meets the technical requirements of the specificataions: and (2) Meets required delivery date as specified in the solicitation; and (3) Past Performance. (b) A written notice of award or acceptance of an offer mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer) whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. Offerors must include a completed copy of the provision at 52.212-3. Only offers received by the closing date will be accepted. QUOTES SHALL INCLUDE THE FOLLOWING: 1. Completed Standard Form 1449. 2. Narrative that addresses how you will meet the Technical Requirements and the Required Delivery date. 3. Provide no less than two contracts in the past 3 years that represent relevant past and present performance. The two contracts that represent relevant past and present performancedocuments must include one Prime Contractor past Performance or one sub-Contractor Past Performance. Vendor must include the following information for each contract Name of Contract, Contract Number, Total Contract Value, Completion Date or state of completion if still in progress. Contractor's Officer's Name, address, telephone, Project location and whether the project was completed timely. Only references for projects of similar size, scope, and complexity are desired. 4. completed pricing schedule (see below) Offeeror Representations and Certifications--Commercial items, with their offer. The clause at 52-212-4, Contract Terms and Condition--Commercial Items, applies to this acquisition. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statues or Executive Orders--Commercial items, applies to this acquisition. Items to be supplied: Twenty Each Model 431-24-66M Tone Remote including 431-RBC-611S 2/4 Wire, 431-RBC-613 Par. TX/Notch filter, 431-Rac-620 Alpha Numeric Display and 431-20-88-M02 Multiline interface box, and any other applicable charges related to delivery of these items.
