- scanners equipment This is a Request for Quotation prepared in accordance with FAR Part 13, as supplemented with additional information included with this notice. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; A SEPARATE WRITTEN SOLICITATION WILL NOT BE ISSUED. QUOTES ARE BEING REQUESTED. The Centers for Medicare and Medicaid Services intends to award using Simplified Acquisition Procedures, a firm fixed priced purchase order the following supplies: (21) - Canon DR-6010C scanner (ship 2 to each RO except 3 to Boston (4) - Canon ScanFront 220P scanner (ship to NY) (4) - Canon P-150 Scanner (ship to NY) (50) - Pick Rollers for Fujitsu fi-5530C (ship 5 to each RO) (10) - SCANAID consumables kit for fi-5530C (ship 1 to each RO) (1) - SCANAID consumables kit for fi-5650C (ship 1 NY) (20) - Exchange roller kit for Canon DR-6010C (ship 2 to each RO) The North American Industry Classification System Code (NAICS) is 333315. The Statement of Work/Deliverables are attached. The period of performance is September 15, 2010 - September 14, 2011. Responses must be sent electronically in MS Word format to Lucille.Lee@cms.hhs.gov no later than 11:00am on July 21, 2010 to Lucille Lee. All responses/quotes submitted shall remain valid for a period of 180 days. For the purpose of establishing timely receipt of a quotation, the time will be established based upon the electronic submission. Please submit proposals in whole dollar amounts only.
