Description
OCCUPATIONAL MEDICAL CLINIC ACQUISITION SYNOPSIS This synopsis serves as a presolicitation notification of the government's intent to issue an RFP and ultimately award a fixed price contract to provide the services summarized herein. This notification will be publicized for 15 days and any subsequent RFP is anticipated to be issued soon thereafter. A. Description of Requirement/Statement of Need: A fully operational Occupational Medicine Clinic at Hill AFB UT, building 249. The clinic will facilitate all services for the civilian population as well as treatment of active duty as deemed necessary. The projection is based on a 23 person clinic consisting of 3 physicians, 1 physician's assistant, 5 nurses, 5 medical assistants, 7 medical records clerks, 1 administrative assistant, and 1 respiratory fit test technician. An Air Force Civilian Occupational Medicine Physician serves and will continue to serve as the Quality Assurance Personnel (QAP). The QAP will have oversight of the clinic to ensure proper performance as directed from HQ AFMC/SGA. A Statement of Work (SOW) that fully outlines job descriptions and qualifications necessary for each position within the clinic will be utilized. B. Government-Furnished Property/Information: The contractor will be allowed use of building 249, Jensen Occupational Clinic, to conduct daily operations. Equipment, including crash carts, medical supplies, immunizations and other materials to maintain the clinic will be provided. Maintenance of the equipment remains with the 75th Medical Group. C. Delivery or Performance Period: The contractor must be able to begin performance 1 October, 2010. The basic period of performance is 1 October 2010 - 30 September 2011. Four option years will follow: 1 October 2011 - 30 September 2012, 1 October 2012 - 30 September 2013, 1 October 2013 - 30 September 2014, 1 October 2014 - 30 September 2015. D. Market Research Results/Interested Sources: Small business goals/targets were addressed in making the decision for full and open competition. It is determined as the best practice of the government to solicit offers from all available sources. Small businesses shall be considered and may be determined as the best value; however, no preference or evaluation adjustment will be made on their behalf, nor will the solicitation/contract be set aside for small business concerns. Extensive research into past performance will be conducted through submittals from the offered contractors, questionnaires from previous customers of the offered contractors, and telecom interviews made by the past performance team during the evaluation process as necessary. The ability and plan to hire proper personnel will also be evaluated as a technical factor under a compensation plan
