- Project: The Intent of this scope of work is to provide construction service t o install Slate tile and to paint in the Centers Male dorm bathrooms Owner: US Department of Labor (DOL) Employment and Training Administration Center Operator : Adams and Associates, Inc Mailing address: 1101 Mills Rd Joliet, IL 60433 Proposal Due Date: July 12 th, 2010 C.O.B (Close of Business) Evaluation Criteria: Cost Experience/Service Quality Payment: The Center will make payment within 30 days of completion. Proposal Requirements Project: The Intent of this scope of work is to provide construction services t o install Slate tile and to paint in the Centers Male dorm bathrooms Owner: US Department of Labor (DOL) Employment and Training Administration Center Operator : Adams and Associates, Inc Mailing address: 1101 Mills Rd Joliet, IL 60433 Proposal Due Date: July 12 th, 2010 C.O.B (Close of Business) Evaluation Criteria: Cost Experience/Service Quality Payment: The Center will make payment within 30 days of completion. Site Visit Date: July 8 th, 2010 @ 10:30am a) Contractor shall visit the site and familiarize himself with the condition under which the work will be performed. No allowance will be made for any error on the contractors part for failure to take into account any conditions or circumstance existing at the site which might affect the contemplate work. The fact that a bid is submitted will be taken as evidence that the contractor has full knowledge of the extent and character of the contemplate work and any problem to be encountered in the performing same. II. Project Description Scope of Project: This project consists of but not limited to the repairs of damage drywall and installation of ceramic tile above and along the sides of the shower stalls to eliminate deterioration. Approximately 2,232Sf of repairs and of wall tile, paint the remaining walls of the showers with a semi-gloss low VOC paint (approximately 3,600 Sf) III Specific Requirements: •a) The contractor shall patch and repair deteriorated drywall and peeling paint to a depth to where all the damage has been removed. •b) Remove and reinstall existing shower rod •c) Install slate tile 18" above shower stalls and cap with a 3" bull nose, along the sides use 3" bull nose to be consist with tile pattern of the shower stalls to eliminate deterioration. •d) Type of tile/grout will be exactly to existing tile (CS53 Continental Slate 6"X6" Asian black, CS50 Continental Slate 6"X6" Egyptian Beige) •e) Patch and repair area as needed on remaining walls •f) Paint the remaining walls with similar color, 1 coat semi-gloss Warrantees: •a) The contractor is to provide written warranty for the above statements at time of completion of job IV Construction Requirements Center Regulations: Contractor and his employees shall abide by all of the Center's rules and regulations. There shall be no fraternization between contractor personnel and Job Corps Center students or personnel. The Contractor will be required to follow all Center safety and security requirements. Management of Jobsite: The Contractor shall maintain a clean and safe workplace that is secured at the end of each working day. The Contractor shall clear all deliveries with the Center's security personnel. Wage Rates: The bidders shall conform to the latest Davis-Bacon prevailing wage rate decisions for the area. Workman's Compensation/Employers Liability insurance, to meet New York statutory requirements Comprehensive General Liability ($1,000,000 per occurrence) Comprehensive Automobile Liability ($1,000,000/combined single limit) Fire Legal Liability ($500,000 per occurrence) The Contractor must cover any deductible costs. Upon contract award, the Contractor must add Joliet Job Corps as additional named insured to the policies. Schedule: The Contractor shall submit a schedule for the work. The project shall be completed within 60 days of the award of the contract. Written authorization is required to extend the 60 day completion requirements. Final Acceptance The Contractor shall notify the Center's maintenance supervisor when the project is substantially complete, and shall request an inspection. The completion of this inspection shall determine the date for the commencement of the warrantees, and the date for payment. V. Proposal Form The Contractor shall submit the following information with the proposal: A list of materials and product data sheets proposed for use. References
