- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotes are being requested and a written solicitation will not be issued. The solicitation number, VA-69D-10-RQ-0425, is issued as a request for quotation (RFQ). This document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-42 and Veterans Affairs Acquisition Regulation (VAAR). The North American Industry Classification System (NAICS) Code for this acquisition is 423450, Size Standard: 100 Employees. This procurement is unrestricted. Offeror's quotes are hereby being requested and shall be submitted on offeror's letterhead or bid form and shall contain the name, title, and signature of person authorized to submit quotes on behalf of the contractor. The following items are known to meet the VA's needs and shall be made in two deliveries prior to September 30, 2010. Description: Item (1) Sterile, Vinyl, Examination Gloves - Small, QTY: 124,200 - Pairs Unit Price per Pair $_______________; CS Price $_______________; Item (2) Sterile, Vinyl, Examination Gloves - Medium, QTY: 237,000 - Pairs Unit Price per Pair $_______________; CS Price $_______________; Item (3) Sterile, Vinyl Examination Gloves - Large, QTY: 360,600 - Pairs Unit Price per Pair $_______________; CS Price $_______________; TOTAL PRICE $_______________; Salient Characteristics: (1) Gloves must be sterile, vinyl, latex-free, and powder-free. (2) Each case shall contain 200-PR for all sizes stated herein. (3) Each pair of gloves shall come in an unopened package and be individually wrapped for single patient use. (4) A shelf-life of 3 to 5 years is required. Expiration date of gloves: The gloves must have 80% of remaining shelf-life. (5) It is critical that each case of gloves takes up no more than 14" x 12.17" x 8" space. (6) All gloves to be delivered in full by September 30, 2010. Delivery shall be FOB DESTINATION to Edward Hines VA Medical Center, 1st Ave - 1 Blk North of Cermack Rd, Hines, IL 60141, Attn: Keisha Greenwood. The Contractor is responsible for all freight charges. The provision at 52.212-1, Instructions to Offeror--Commercial Items, applies to this solicitation. Award will be made to the offeror representing the best value to the Government. Best value will be determined by ability to meet salient characteristics, price, past performance, and service disabled veteran owned or veteran owned small business status. Quotes will be evaluated in accordance with FAR 52.212-2 Evaluation-Commercial Items using the evaluation factors of Ability to meet Salient Characteristics, Price, Past Performance and Service Disabled Veteran Owned or Veteran Owned Small Business Status. The provision at 52.212-3, Offeror Representations and Certification--Commercial Items, applies to this solicitation. The contractor shall return a completed copy of this provision with its quotation. The resulting firm fixed price award will incorporate the requirements of the clause at FAR 52.212-4, Contract Terms and Conditions--Commercial Items. The Offeror shall also provide its DUNS number and Tax Identification number. All Contractors, both large and small, are required to register in the Central Contractor Registration (CCR) database before an award.
