This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, proposals are being requested and a written solicitation will not be issued. The solicitation number is VA-256-10-RQ-0213. It is the contractors ™ responsibility to be familiar with the applicable clauses and provisions. Both may be accessed in full text at www.acquisition.gov. This is a 100% Small Business Set-Aside. The North American Industry Classification System (NAICS) Code for this acquisition is 811219. CLIN 0001 1 JB TOTAL: $____________________________ The contractor shall furnish all necessary labor, parts, transportation and equipment to provide calibration certifications on Government-owned equipment listed below located at the Overton Brooks VA Medical Center (OBVAMC), located at 510 East Stoner Ave, Shreveport, LA 71101 and Texarkana Community Based Outpatient Clinic (COB), located at 2717 Summerhil Road, Texarkana, AR. In accordance with all terms, conditions, provision, schedules and specifications of the solicitation form October 1, 2010 through September 30, 2011, pending availability of funds. Payment will be made monthly in arrears upon receipt of a proper invoice at the office designated to receive the invoice. Audiometers requiring semi-annual calibration certification inspections (October, April): Associated audiometer booth certifications will be performed with each audiometer check: Location: OBVAMC: Audiometer; Welch Allyn; Model: GSI-61; Serial No: 981651. EE# 20818 Audiometer; Grayson Stadler; Model: GSI-61; Serial No: 20030757. Location: Texarkana CBOC: EE# 25677 Audiometer; Maico Hearing Insts. Model MA-41 Serial No: 69883. Location: BW12; EE# 18716 (portable audiometer, no booth check required). Vendor will provide two on-site calibrations to meet ANSI or manufacturer's specifications, whichever is more stringent. The Contractor will coordinate all maintenance visits through the COTR or other Biomedical Engineering representative. Upon completion of work, a copy of the service report will be provided to Biomedical Engineering. Failure to do so may delay payment. All work shall be accomplished during normal duty hours 8:00 am to 4:30 pm, Monday through Friday, excluding federal holidays. The following are federal holidays: New Year's Day, Martin Luther King Jr.'s Birthday, President's Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veteran's Day, Thanksgiving Day and Christmas Day. Only new standard parts (manufactured by the maker of the equipment or equal thereto) shall be furnished by the contractor. Cellular phones shall not be used in critical areas of patient care. If in doubt, check with the Contracting Officer or the Contracting Officer's Technical Representative. The Overton Brooks VA Medical Center is a smoke free facility. Smoking is prohibited inside any building except the smoking shelter on the facility grounds. Smokers are allowed to smoke outside the 25 foot boundaries from every entrance/exit. Violators shall be removed from the premises permanently. The instructions shall also contain a complete list of all replaceable parts showing part number, name and quantity required; 852-211-71, Guarantee (NOV 1984) “ The contractor guarantees the equipment against defective material, workmanship and performance for a period of said guarantee to from date of acceptance of the equipment by the Government. The contractor agrees to furnish, without cost to the Government, replacement of all parts and material which are found to be defective during the guarantee period. Replacement of material and parts will be furnished to the Government at the point of installation, if installation is within the continental United States or f.o.b. the continental U.S. port to be designated by the contracting officer if installation is outside the continental United States.
