- Request for Quotation Form Terms and Conditions-Asphalt COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS (IAW FAR 12.603 (c)) (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotes are requested and a written solicitation will not be issued. The potential award as a result of this solicitation will be a Blanket Purchase Agreement (BPA) with a 5-year period of performance and in accordance with the attached Schedule. (ii) This synopsis/solicitation reference number is FA4418ASPHALT and is being issued as a Request for Quotation (RFQ). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-41 dated 13 May 10. (iv) This acquisition is 100% set-aside for small businesses. The associated NAICS code is 324121 with a size standard of 500 employees. (v) Interested contractors are invited to submit a quote for asphalt, binder, superpave, tack, prime, crushed concrete, RAPP (crushed millings) as indicated on the attached Request for Quotation Form. "Brand Name Or Equal" items will be accepted. (vi) The government will pick up materials that will be ordered under resultant BPA. Due to this fact, the contractor's pick-up area must be located within 20 miles of Charleston AFB. (vii) The following clauses and provisions and attached terms and conditions are incorporated and will remain in full force in any resultant award: (full text of clauses and provisions may be accessed electronically at this website: http://farsite.hill.af.mil) FAR 52.212-1 Instructions to Offerors-Commercial. (viii) FAR 52.212-2 Evaluation-Commercial Items Evaluation. The Government will award a BPA resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the government, price and other factors considered. The following factors shall be used to evaluate offers: (1) Price and (2) technical capability of the item offered to meet the government's requirement. The government will evalaute offers for award purposes for the total price of the requirement. Please use the attached Request for Quotation form to submit pricing. (ix) All offerors shall include a completed copy of FAR 52.212-3, Offeror Representation and Certifications--Commercial Item or complete electronic annual representations and certifications at http//orca.bpn.gov.
