EXHIBIT E - PROPOSAL VOLUME CONTENTS AID EXHIBIT C PACKAGE INFORMATION AND FORMS EXHIBIT B – SUBCONTRACTOR/TEAMING PARTNER/KEY PERSONNEL INFORMATION AND CONSENT FORM EXHIBIT A PERFORMANCE RELEVANCY QUESTIONNAIRE PWS Solicitation This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; Proposals are being requested and a written solicitation will not be issued. The Solicitation Number is W91ZRS-10-R-0005, Request for Proposal. The North American Industry Classification System (NAICS) Code is 336413; size standard is 1000 Employees. This acquisition is reserved for small Business concerns. The Government anticipates award of a firm fixed price contact. The Connecticut Air National Guard requires a T-9 Hush House Relocation that consists of: 1. Disassembly of A/F32T-9 Hush house presently located in Bangor International Airport, Maine. 2. Shipping of all content and components to 103AW East Granby, Connecticut 3. Repair and refurbishment of A/F32T-9 4. Assembly of A/f32T-9 on foundation/pad located at 103AW East Granby, Connecticut This requirement consists of furnishing all labor, expertise, equipment, materials, facilities, transportation, and appliances necessary to relocate,(disassemble, transport, repair/refurbish, and assemble) a fully operational A/F32T-9 Hush house in accordance with but not limited to the statement of work, technical Orders, and Drawings. **Please note that drawings may not be all inclusive, and /or may contain a control room which is not necessary for this requirement***. This requirement must comply with all the requirements set forth by the Air Force (AF), National Guard Bureau NGB),federal, state, and local regulations. Proposals will be evaluated as a Best Value Judgment, and proposals should be sufficiently detailed in order to make that determination. Please see attached document for details of requirements. All proposals will be considered. The Government anticipates a firm fixed price, single award contract from this solicitation. Site visit will be conducted on 16 June 2010 at 1000 at Bangor International Airport, Maine. All individuals wishing to attend must pre-register no later than 14 June 2010 with the following information: Solicitation number, date and time of site visit, Firm/Company name and telephone number, Visitor's Name and Driver's License number for each attending individual, vehicle description to include make, model, year and color (indicate if rental). Interested parties are highly encouraged to attend the site visit. The contractor is required to acknowledge at the time of closing that the contractor has inspected and is fully cognizant of site conditions. No allowance or consideration will be given after award for condition(s), which existed at the time of the site visit. The government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the government. The following Federal Acquisition Regulations (FAR) and its supplements DFAR and AFAR provisions and clauses apply to this acquisition; 252.204-7004, Required Central Contractor Registration, 52.212-1, Instructions to Offerors -Commercial Items, 52.212-4 Contract Terms and Conditions - Commercial Items; 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items, 52.232-33 Payment by Electronic Funds Transfer. The Government anticipates award of a firm fixed price contract. To receive an award of contract the offeror must be registered in the Central Contractor Registration (CCR) database which can be accessed at the website http://www.ccr.gov or call Assistance Center toll free at 1-888-227-2423. Interested offerors are required to submit their representations and certifications on-line at http://orca.bpn.gov. You will need your DUNS number and CCR number in order to register to the Online Representations and Certifications Application (ORCA) website. Offers shall be submitted no later than 12:00PM, 30 June 2010, to the Connecticut Air National Guard, Purchasing and Contracting Office, ATTN: Dawn Whelan, 100 Nicholson Rd, East Granby CT, 06026.
