Description
Statement of Work This is a combined solicitation/synopsis for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a Request For Quotation (RFQ), solicitation in accordance with FAR Parts 12 and 13. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC 2005-40 effective 22 Apr 2010). The North American Industry Classification System (NAICS) number is 562111 with size standard as $12.5 Million. The requirement is not set-aside for small businesses, but small businesses, woman-owned, and service disabled veteran-owned businesses are encouraged to participate by submitting quotes. It is incumbent upon the interested parties to review this site frequently for any updates/amendments to any and all documents. The needed service is as follows: CLIN 0001 Recycling Services 12 Months x ____Unit Price = _________________ DESCRIPTION OF SERVICES. The Contractor shall provide all personnel, equipment, tools, materials, vehicles, supervision, and other items and services necessary to perform recycling services at Barksdale AFB, LA. The Contractor shall perform to the standards in the contract as well as all local, state, and federal regulations. See attached Statement of Work for specifics on this requirement. Period of Performance: 1 October 2010 thru 30 September 2011 CLIN 1001 Recycling Services 12 Months x ____Unit Price = _________________ DESCRIPTION OF SERVICES. The Contractor shall provide all personnel, equipment, tools, materials, vehicles, supervision, and other items and services necessary to perform recycling services at Barksdale AFB, LA. The Contractor shall perform to the standards in the contract as well as all local, state, and federal regulations. See attached Statement of Work for specifics on this requirement. Period of Performance: 1 October 2011 thru 30 September 2012 CLIN 2001 Recycling Services 12 Months x ____Unit Price = _________________ DESCRIPTION OF SERVICES. The Contractor shall provide all personnel, equipment, tools, materials, vehicles, supervision, and other items and services necessary to perform recycling services at Barksdale AFB, LA. The Contractor shall perform to the standards in the contract as well as all local, state, and federal regulations. See attached Statement of Work for specifics on this requirement. Period of Performance: 1 October 2012 thru 30 September 2013 Offerors must comply with all instructions contained in FAR 52.212-1, Instructions to Offerors Commercial Items, applies to this acquisition and the following addenda applies. Offerors shall submit their prices on letterhead stationery annotating Cage Code Number, DUNS Number and Tax Identification Number. THE GOVERNMENT WILL NOT ACCEPT INCOMPLETE PROPOSALS. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforms to the solicitation that will be most advantageous to the Government. The Contracting Officer and the Contract Administrator are not responsible for locating or obtaining any information not identified in the offer. If an offeror cannot comply with every requirement, that offer will not be acceptable, and therefore will not be considered. Offers will be evaluated on price based and upon technical capability, however, the Contracting Officer reserves the right to award to other than the lowest quote based upon a best value determination by the Government. Before an award can be made, according to DFARS 252.204-7004 Alt A, offeror must be in the Contractor Central Registration (www.ccr.gov) database and also in the Online Representation and Certifications database (https://orca.bpn.gov/). Additionally, the following clauses and provisions apply to this solicitation: 52.204-7, Central Contract Registration 52.204-9, Personal Identity Verification of Contractor Personnel 52.212-4, Contract Terms and Conditions Commercial Items 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items, and includes the following clauses as cited: (b)- (8)(i), (19), (21), (22), (23), (24), (25), (33), and (40); (c)-(1), (2) In compliance with Service Contract Act of 1965, as amended, and the regulations of the Secretary of Labor (29 CFR Para 4), this clause identifies the classes of Service employees expected to be employed under this contract and states the wages and fringe benefits payable to each if they were employed by contracting agency subject to the provisions of 5 U.S.C. 5351 or 5332. This statement is for information only, it is not a wage determination.
