Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, and 13.106, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (III) The provisions and clauses incorporated herein are those in effect through Federal Acquisition Circular 2005-23, effective December 26, 2007. This requirement is being synopsized as full and open competition under standard industrial classification code 8748, NAICS Code 611430, size standard is $7.0 million. The resulting contract will be a Firm Fixed Price type Contract under FAR Part 12 - Acquisition of Commercial Items. COMMERICAL ITEM DESCRIPTION: The Department of Veterans Affairs Medical Center/Employee Education Resource Center, St. Louis, MO proposes to enter into a firm-fixed contract for registration fees for VA Facility Members in the Institute for Healthcare Improvement program. Proposals are due Tuesday May 18, 2010 by 1:00 pm CST to Linda Eley, linda.eley@va.gov. Any other official may disqualify the offeror from submission of a quote. 1.Scope The purpose of this is for registration fees for participation of Veteran Affairs (VA) facility staff members in the Institute for HealthCare Triple Aim Program. 2. Background Veteran Health Administration (VHA)'s continued participation in this initiative in conjunction with other leading healthcare systems from all over the world provides VHA the opportunity to continue the work that began in the earlier phases and to address healthcare improvement issues and apply the most contemporary innovations in healthcare delivery. The teams that participated in previous phases will continue their attendance at this program that began in previous phases and new teams will be identified to join. The current VA teams along with the new teams will be joining this initiative to develop, test, and implement triple aim innovations around selected populations. Triple aim, in short, is about improvement in the health of populations, cost and healthcare experience. Teams will set local aims for improvement, connect with ideas from other systems around the world, and test these changes within important VA populations: Patients with Congestive Heart Failure and Chronic Lung Disease (which account for large numbers of hospital admissions) and patients with Diabetes who live in rural and highly rural areas. These teams will benefit from learning approaches to improvements attempted by other healthcare systems. In addition, the VA teams will share their experiences and innovations with the other teams in the wider world of healthcare. 3. COTR Josh Smith, Education Specialist Long Beach Employee Education Resource Center VA Medical Center 5901 E. 7th Street Long Beach, CA 90822 Phone: 562-826-5505 FAX: 562-826-5986 4.0 Tasks 4.1 Learning calls for participants Contractor shall conduct 3 conference calls per month 1st and 3rd Tuesday of each month at 11 am Eastern Time and 2nd Tuesday of each month at 10 am Eastern Time. Calls will be lead by the vendor. Conference call lines will be arranged by VHA staff using the VANTS system. Agenda for calls and content development will be completed by the vendor. Content to be discussed on each call will be planned in conjunction with the Systems Redesign (SR) Office staff. 4.2 Face to Face Programs Contractor shall conduct face to face meetings 4.3 Site visits with faculty 4.4 Participation in selected research and development projects Participants will be afforded the opportunity to participate in a variety of research projects. 5.0 Performance Measure Task 4.1 Learning calls for participating teams Deliverable: Contractor shall plan and conduct learning calls on the specified dates and time. Vendor facilitates the conference calls and develops the agenda for each call in collaboration with SR Office designee. Agendas must be submitted for approval at least 3 work days prior to the conference call. Delivery timeline: After execution of the contract, the calls will begin immediately and on the 1st and 3rd Tuesday of each month at 11 am Eastern Time and on the 2nd Tuesday of the month at 10 am Eastern time.. Performance Measure: Calls will occur 100% of the time at the agreed upon dates and times. All calls will have agendas and/or materials for participants. Calls will start on time and will be conducted for the agreed upon length of time (minimum 50 minutes). Means of Measurement: SR Office staff will participate on all calls and assess vendor's compliance. 4.2 Face to Face Programs Deliverable: 1 virtual conference will be conducted and 1 face to face program will be conducted. Contractor shall develop program agenda in collaboration with the SR Office staff, prepare any handout or resource materials for the programs, notify participants of dates and times of program and make all arrangement and plans for the programs including program arrangements and logistics including identifying and handling of venue. Delivery timeline: Dates of virtual program and face to face program will be determine in collaboration with SR Office staff. All arrangements and logistical information must be completed not later than 6 weeks prior to the program date and program information sent to VA participants not later than 6 weeks prior to the program.. Performance Measure: Virtual conference and face to face conference held on the dates agreed upon by contractor and VA SR Office staff. All planning materials and program logistical information send to VA SR Office staff by agreed upon timeline (not later than 6 weeks prior to the program). 100% of participants receive all program resource and handout materials. Contractor shall complete participant program evaluations and provide results to VA SR office within 10 days after the program. Program evaluations should be on average of 3.5 or higher on a 5.0 likert scale. Means of Measurement: Report from SR Office staff and program evaluations. Evidence of date of notification of program logistics will be completed via the date of the email sent to VA participants. 4.3 Faculty visit participant VA sites and conduct site visit. Deliverable: faculty site visit to participating team facilities. Agenda for site visit attached t this SOW. Delivery timeline: Site visits dates will be planned by the contractor in collaboration with the SR Office designee and the field facility staff.. Performance Measure: 100% of site visits completed on the timeline agreed upon with the SR Office. Contractor shall conduct evaluation of the site visit experience to determine its value in the completion of the goals of the Triple Aim program. Means of Measurement: Report from the SR Office staff. Evaluation result share with SR Office staff. 4.4 Participation in selected research and development projects Deliverable: VA participants will be offered the opportunity to participate in special research and development projects with contractor. Delivery timeline: Contractor shall solicit interest from VA participants prior to completion of all components of the Triple Aim program.. Performance Measure: Contractor to notify participants prior to 11/2010 Means of Measurement: Notification to SR Office. 6. Period of Performance This program will begin immediately after awarding of contract up until November, 2010 5. Deliverables Triple Aim Teams will help VHA meet three critical objectives: "Improve the health of the target populations for each respective team; "Enhance the patient experience of care (including quality, access, and reliability); and "Reduce, or at least control, the per capita cost of care. 6. Evaluation Factors a.Past performance- A critical factor in this contract is previous performance presenting Triple Aim content, preferably to VA audiences and for certain to other leading health care organization. b.Five years or more experience in conducting training on healthcare improvement with large complex healthcare systems. c.Experience conducing training with large healthcare systems on Triple Aim concepts. d.National or international recognition as a leader in healthcare improvement training. e.Knowledge of an experience training executive staff from large complex healthcare systems. 7. Proprietary Information Statement The government will retain rights to all documents produced with this contract. 8. Security Requirements Contractor shall have access to Privacy Act information and will be expected to sign a "Statement of Responsibility and Rules of Behavior for the Employee Education System (EES) Automated Information Systems (AIS)". Contractor shall be required to take and pass all applicable information security and Healthcare Information Privacy Policy Act (HIPPA) courses as requested.
