- COURIER SERVICE For 3rd MEDICAL GROUP HOSPITAL 1. GENERAL: Contractor shall provide all labor, material, equipment, transportation, and certification/licensure for transportation of medical samples to local hospitals and other businesses deemed necessary. 2. SERVICE LOCATION: 3rd Medical Group 5955 Zeamer Ave. Elmendorf AFB, AK 99506 3. PERFORMANCE PERIOD: 1 Jul 2010 through 30 Jun 2013. With yearly option to renew services. 4. SERVICE HOURS: The contractor will be available for delivery 24/7, 365 days a year. 5. DESCRIPTION: A courier service is required to deliver and pick-up to and from various medical facilities to include but not limited to Blood Bank of Alaska, Providence Hospital, and Alaska Regional Hospital. Service needs to be available anytime day or night 365 days a year. We require the courier to be here for pick-up within 20-30 minutes from the time a service call is placed. 6. RESPONSIBILITY OF CONTRACTOR: The contractor shall maintain all Federal and State requirements/licensure as required. 6.1 Medical specimens in transport shall be safeguarded according to Center for Disease Control and Prevention and Prevention and Dept. of Transportation regulations governing shipments of diagnostic/infectious substances. 6.2 All drivers shall have specific training/supplies to handle cleanup of spills (Hazmat) etc associated with medical specimens. 6.3 All couriers must be eligible to enter Elmendorf AFB and be current on entrance requirements. 6.4 Driver personnel records with documentation of required training will be furnished to the 3 MDG Laboratory Flight within 14 days of request. 6.5 Return all shipping boxes and other reusable packing materials to the 3 MDG Laboratory Flight on next routing scheduled pick up. 6.6. Adhere to Alaska's Department of Labor Wage Determination Rate 7. FINANCE: 7.1 A copy of the invoice with all charges will be submitted no later than 5 business days from the last day of the month. 8. RESPONSIBILITY OF 3RD MEDICAL GROUP 8.1 Provide originating caller name, contract phone number and location (i.e. laboratory, pharmacy, operating room etc) to ensure packages are ready for immediate pick up. 8.2 Provide all packing materials including shipping containers and ensure that all packages are correctly addressed. 8.3 Ensure all packages are prepared or packed according to International Air Transportation Regulations (IATA) packing instruction 650 or 602 for diagnostic or infectious substances as appropriated to the contained specimens, etc. 8.4 Ensure sufficient amounts of wet or dry ice are packed with the specimens if required to provide the proper storage temperature for the duration of the transportation process. 8.5 Provide a contact name/number for both the sender and receiver. 8.6 Be available 24 hours/7 days a week to answer technical questions assist with questions concerning the nature of specimens shipped. 8.7 Coordinate access to Elmendorf Air Force Base according to current Security Forces rules, regulations and requirements. 9. QUALITY CONTROL 9.1 The contractor shall develop and implement a plan to monitor the quality and timeliness of services provided under this SOW. 10. SAFETY 10.1 The contractor and/or representatives shall comply with the following Elmendorf Air Force Base safety standards at all times. 10.2 Wearing seatbelts/shoulder harness and observing posted base traffic regulations and maximum speed limits while driving on Elmendorf AFB, AK. 10.3 Refrain from using cellular telephones while operating a motor vehicle on Elmendorf AFB, AK. 11. WORK LOAD ESTIMATE 11.1 Services will be requested on an "as needed" basis. Previous delivery requirement estimates are 20-30 service calls per calendar month.
