- The Department of Veterans Affairs Medical Center (VAMC), VA Medical Center in Fayetteville, North Carolina is seeking potential sources capable of providing Community Based Health Care for Homeless Veterans (HCHV), inclusive of all services as may be necessary in the treatment of patient care. Transitional housing for the type of patient to be cared for under this contract will normally require care and services over and above the level of room and board. Homeless Veterans may have ongoing Mental Illness, Depression, Schizophrenia, Post Traumatic Stress Disorder, and/ or Substance Dependencies. The facility will not be used for detoxification or other hospital level treatment, VAMC Salem will service as treatment center. The contractor shall furnish each veteran authorized care under this contract, housing, to include, but not necessarily limited to, beds, restroom, bath, and kitchen facilities, and related furnishings. Adequate security measures, e.g., bars on windows and doors and area fenced, in order to provide optimum safety for residents. Provide food for two balanced meals daily and snacks daily to each shelter resident. Contractor to provide personal hygiene items, first aid supplies, and storage as needed. Room and board to include a bed and other furnishings such as a dresser, storage, and linen for the bed. Provide access to a kitchen for preparation of two meals (breakfast and supper) a day, 7 days a week and availability of nutritious snacks between meals and bedtime for those requiring or desiring additional food, when it is not medically contraindicated. Provide a laundry facility for residents to do their own laundry or to have laundry done. The total number of beds required is five (5). No patient will stay beyond the maximum time of 4 months. The period of performance is from July 1, 2010 (or date of award) through Oct 31, 2010 with four (4) one (1) year renewal optional periods that can be exercised by the Government. The NAICS code for this requirement is 623220. Size standard is $7.0 Million. This is a sources sought notice only. No formal solicitation document exists at this time. Responses to this notice should include the following: name and address of company; business size (large or small); identify whether or not firm falls into any of the following socioeconomic categories “ 8(a), HUB Zone, Service Disabled Veteran-Owned; firms experience in providing services.
