- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. SAMMC, Fort Sam Houston Texas, has identified a requirement to provide 12 Metal/Granite Benches which are one of a kind items and reflect the project vision statement for the FSH clinic that captures the history, culture, colors and locale of the San Antonio area and enhances the healing environment of the clinic which serves the military and beneficiary population. Additionally, one original art metal wall plaque which is a one of a kind item that captures the history, culture, colors and locale of the San Antonio area and enhances the healing environment of the clinic which serves the military and beneficiary population. The contractor/artist shall provide materials, labor, equipment and installation required to perform all work to conform to the terms, conditions and specifications of the Statement of Work (SOW). This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation shall not be issued. Solicitation W9124J-10-T-0031 is issued as a request for quotation. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 05-40. This is a 100% small-business set-aside for firms that provide services under NAICS code 327991 (Cut Stone and Stone Products Manufacturing) with a business size of 500 Employees. CONTRACT LINE ITEM NUMBER (CLIN) 0001. -Metal/Granite benches, 12 Each. Total ____________. -Firm Fixed Price/Procurement shall include delivery and installation by the vendor of 12 original art metal benches. See Statement of Work. Art Metal Benches are cut with CNC plasma out of 3/8 inch steel plate and have 5 cm/2 inch granite seats; benches measure 72 inches long, and the back of the bench 48 inches to 54 inches; bench seat varies between 17 inches to 19 inches high. Actual size of bench depends upon the design of the bench. Each bench weighs approximately 500 lbs. All metal art benches will be sand blasted and will have a powder coated finish. Metal benches are one of a kind items and reflect the project vision statement of Ft. Sam Houston Clinic. -Contractor to deliver and install all benches Not Later Than March 31, 2011, with the first three benches delivered and installed Not Later Than July 31 2010. CLIN 0002. -Wall Metal Artwork, 1 Each. Total ____________. -Firm Fixed Price/Art metal plaque is cut 3/8 inch (10 gauge) steel plate with hand forged pieces and shall be sand blasted and powder coated. Plaque measures approximately 36 inches wide and 40 inches high. See Statement of Work. -Contractor to deliver and install wall metal artwork to coincide with delivery of first three benches (Not Later Than July 31, 2010) at the Main corridor, 1st Level of the FSH Health Clinic. STATEMENT OF WORK Original Art Metal/Granite Bench Artwork and Metal Wall Plaque Artwork 1. General 1.1. Contractor shall furnish and install 12 original metal/granite benches with metal artwork and one metal wall plaque. All artwork shall be one of a kind items and reflect the project Vision statement for the Fort Sam Houston Clinic 2. Scope of Work. 2.1. Contractor/Artist to deliver and install 12 original art metal and granite benches to: 2.1.1. Location: Benches 1 through 3 (Historic Alamo Mission; Historic Concepcion Mission; and Historic Espada Mission) shall be placed on 1st Level of the FSH Health Clinic, exterior walkway adjacent to Main Entrance. Benches 4 through 12 (Texas State Capitol; Texas Covered Wagon; Texas Lone Star; Texas Windmills; Texas Rodeo; Texas Square Dancing; Texas State Bird (Mockingbird); Texas Longhorn; and Texas Cowboy Hat, Boots & Spurs) shall be placed in the 1st Level of the SAMMC-North Consolidated Tower (exterior walkways leading into main entrance and exterior courtyard). 2.2. Description : Overall color palette: Metal works to have Antique Copper paint color; granite bench seat shall be light in color (such as Amber Gold/Cold Spring Granite). 2.2.1. Materials: Shall be 3/8 inches steel plate for frame and back; granite shall be 5 centimeters (cm)/2 inches (approximately). 2.2.2. Production: The metal benches shall be cut out of 3/8 inch steel plate by the CNC plasma method, sand-blasted and have a powder-coated finish. The back of each bench to pictorially depict one of the 12 missions listed below. The granite slab seat shall have smooth edges, shall have adequate understructure/support to prevent cracking and shall be appropriately affixed to the sculptured metal frame. 2.2.3. Size: Each metal bench shall be 6 long x 48 inches to 54 inches high; bench seat varies between 17 inches to 19 inches high and will weigh approximately 500 lbs each. 2.2.4. Installation: All pieces are freestanding and will be located/affixed as noted above. Theme: Metal/granite Benches are one of a kind items and reflect the project Vision statement for the FSH clinic, captures the history, culture, colors and locale of the San Antonio area, and enhances the healing environment of the clinic which serves the military and beneficiary population. -Appropriate/Acceptable Theme for Bench Back #1: Historic Alamo Mission sculpture metalwork image of the faade of the Alamo Mission to be depicted in contoured bench back; Back is to have a cut-away effect (shall not be solid sheet of metal) but shall be sculptural, depicting both positive and negative space. Metal bench back and frame shall have smooth edges with powder-coated finish. There are to be no people shown in the image. Bench seat to be of 2 inches/5 cm thick (minimum) granite stone slab situated on decorative metal framework. Legs of benches are to incorporate one or more architectural motifs of the Alamo Mission (windows, arches, pediment, etc). -Appropriate/Acceptable Theme for Bench Back #2: Historic Concepcion Mission - sculpture metalwork image of the faade of the Conception Mission to be depicted in contoured bench back; Back is to have a cut-away effect (shall not be solid sheet of metal) but shall be sculptural, depicting both positive and negative space. Metal bench back and frame shall have smooth edges with powder-coated finish. There are to be no people shown in the image. Bench seat to be of 2 inches/5 cm thick (minimum) granite stone slab situated on decorative metal framework. Legs of benches are to incorporate one or more architectural motifs of the Concepcion Mission (windows, arches, pediment, etc). -Appropriate/Acceptable Theme for Bench Back #3: Historic Espada Mission - sculpture metalwork image of the faade of the Espada Mission to be depicted in contoured bench back; Back is to have a cut-away effect (shall not be solid sheet of metal) but shall be sculptural, depicting both positive and negative space. Metal bench back and frame shall have smooth edges with powder-coated finish. There are to be no people shown in the image. Bench seat to be of 2 inches/5 cm thick (minimum) granite stone slab situated on decorative metal framework. Legs of benches are to incorporate one or more architectural motifs of the Texas State Capitol (dome, cornerstone, arch, pediment, etc). -Appropriate/Acceptable Theme for Bench Back #5: Texas Covered Wagon - sculpture metalwork image of one or more images of a Texas covered wagon, to include at least one item used on a wagon trail (such as cookware, campfire, etc) to be depicted in contoured bench back; Back is to have a cut-away effect (shall not be solid sheet of metal) but shall be sculptural, depicting both positive and negative space. Legs of benches are to incorporate one or more elements of a Texas wagon or items that may be used on a wagon trail (wagon wheel, wagon hardware, cookware, campfire, etc). -Appropriate/Acceptable Theme for Bench Back #6: Texas Lone Star - sculpture metalwork image of the lone star imposed on top of an image of the state of Texas, to include verbiage that readsLone Star State to be depicted in contoured bench back; Back is to have a cut-away effect (shall not be solid sheet of metal) but shall be sculptural, depicting both positive and negative space. Metal bench back and frame shall have smooth edges with powder-coated finish. There are to be no people shown in the image. Bench seat to be of 2 inches/5 cm thick (minimum) granite stone slab situated on decorative metal framework. Legs of benches are to incorporate one or more images of the Lone Star state (star, image of the state of Texas, Texas state flag, etc). -Appropriate/Acceptable Theme for Bench Back #7 Texas Windmills - sculpture metalwork image of vintage wooden Texas windmills on the West Texas landscape, to include images of fencing and one or more trees, to be depicted in contoured bench back; Back is to have a cut-away effect (shall not be solid sheet of metal) but shall be sculptural, depicting both positive and negative space. Legs of benches are to incorporate one or more images associated with Texas Longhorn cattle (branding iron, silhouette of steer head with horns, Texas star, cactus, etc). -Appropriate/Acceptable Theme for Bench Back #12 Texas Cowboy Hat, Boots & Spurs - sculpture metalwork images of cowboy hat along with cowboy boots with spurs, to include verbiage that reads Cowboy Life; to be depicted in contoured bench back; Back is to have a cut-away effect (shall not be solid sheet of metal) but shall be sculptural, depicting both positive and negative space. Metal bench back and frame shall have smooth edges with powder-coated finish. Bench seat to be of 2 inches/5 cm thick (minimum) granite stone slab situated on decorative metal framework. Legs of benches are to incorporate one or more images associated with cowboys (spurs, stirrups, lariat, etc). 2.4 Original Art - Metal Wall Plaque: 2.4.1. Quantity: 1 original metal wall plaque. 2.4.2. Location: Main corridor, 1st Level of the FSH Health Clinic. Overall color palette: Suggested paint color antique copper. 2.4.3. Appropriate/Acceptable Themes: Tree of life theme reflecting Army families and military structure that supports them, verbiage to be determined, located below tree (may include name of facility). 2.4.4. Materials: 10 gauge (3/8 inch) steel plate. 2.4.5. Production: Shall be cut out of 10 gauge (3/8 inch) steel cut by the CNC plasma method. About 200 leaves (also of 10 gauge steel) shall be hand forged and attached to give 3D effect. Size: 5' wide x 7' high - no more than 4 inch depth. Weight: Not to exceed 50 lbs. 2.4.6. Installation: The entire piece shall be permanently attached to the wall as designated, at a location, by the building manager. 3. Government Furnished Property. 3.1. None 4. Contractor Furnished Material. 4.1. The contractor/Artist shall provide experienced personnel to perform the required services. The Government and the contractor/Artist understand and agree that the services to be delivered are non-personal services and both parties recognize and agree that no employer-employee or master servant relationships exist between the Government and the contractor/Artist and/or between the Government and the contractors employees. Contactor personnel performing under this contract shall not be placed in a position where they are employed by a Federal Officer or are under the supervision, direction, or evaluation of a Federal officer, military or civilian. 4.2. Contractor personnel shall conform to standards of conduct and code of ethics, which are consistent with those applicable to Government employees as provided in the Joint Ethics Regulation 5500.7.R. 4.3. The Contractor shall comply with all applicable laws, rules and regulations, including but not limited to those included in the Federal Acquisition Regulation. 4.4. Contractor/Artist personnel will obtain a vehicle pass for access to Ft. Sam Houston. Date(s) and place(s) of delivery and acceptance and FOB point. See SOW. 4.5. Wide Area Work Flow (WAWF) is the preferred method to electronically process vendor request for payment. This application allows DOD vendors to submit and track Invoices and Receipt/Acceptance documents electronically. Contractors using WAWF shall (i) register to use WAWF-RA at https://wawf.eb.mil and (ii) ensure an electronic business point of contact (POC) is designated in the Central Contractor Registration site at http://www.ccr.gov within ten (10) calendar days after award of this contract/order. Provisions at 52.204-7, Central Contractor Registration. Provisions at 52.204-9, Personal Identity Verification of Contractor Personnel. Provisions at 52.209-6, Protecting the Governments Interest When Subcontracting with Contractors Debarred, Suspended, or Proposed for Debarment. Award will be made to the company with the lowest price (that is reasonable and realistic); that is technically acceptable. This is a 100% small-business set-aside. In order to be technically acceptable the offeror must demonstrate the following factors: -Connection and/or awareness to the one of a kind items and reflect the project Vision statement for the San Antonio Military Medical Center North Fort Sam Houston Clinic, where the benches and metal artwork shall capture the history, culture, colors and locale of the San Antonio area and enhances the healing environment of the clinic which serves the military and beneficiary population. -Appropriateness of artwork to the site. -Ability to meet safety, accessibility and technical requirement of the facility. -These factors can be demonstrated by: 1. Providing a narrative of the proposed artwork. 2. Sketches or illustrations or media presentations of the artwork. 3. Biography of the artist (brief). -The offeror must submit with their proposal the above three factors as these factors demonstrate technical capability. Offerors are to provide a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition and there are no addenda.
