Description
The Department of Veterans Affairs Medical Center, 1970 Roanoke Boulevard, Salem, Virginia is seeking potential sources capable of providing Pharmacy service for Community Based Outpatient Clinics (CBOC) for eligible veterans beneficiaries as referred by the Salem VAMC's (CBOCs) a part of VA Medical Center, Salem, VA. Services are estimated to be needed for a period of one year with Four one year renewable option periods. Pharmacy shall be able to provide prescriptions to VA beneficiaries as prescribed by physicians assigned to the proposed Veterans Affairs Community Based Outpatient Clinics (CBOCs). The CBOCs are located at Tazewell Family Physicians, 123 Ben Bolt Avenue, Tazewell, VA, Valor Healthcare supporting the Danville, Hillsville, VA area, Lynchburg VAMC operation at 1600 Lakeside Drive, Lynchburg, Virginia, Virginia area, and future sites including Staunton and Wytheville, Virginia. The contractor must possess all licenses, permits, accreditation's and certificates as required by law. The contractor shall provide all necessary equipment and personnel to perform Pharmacy Services in accordance with the contract requirements. An estimated 3,000 prescriptions per year or 250 per month will be filled on an as needed basis. Services to be provided by the Pharmacy vendor includes, but is not limited to, medication profile review (screening for allergies, drug-drug interactions, dosage range, dosing intervals and patient instructions), labeling, dispensing and medication use counseling. The NAICS code for this requirement is 446110 / Pharmacies and Drug Stores, Size Standard is $7.0M. The period of performance is from August 01, 2010 (or date of award) through July 30, 2010 with four (4) one (1) year renewal options that can be exercised by the Government. No formal solicitation document exists at this time. Responses to this sources sought notice should include the following: name and address of company; business size (large or small); identify whether or not firm falls into any of the following socioeconomic categories - 8(a), HUBZone, Service Disabled Veteran-Owned; firm's experience in providing local HCHV Services meeting VAMC requirements in a professional setting and any other pertinent information, which demonstrates ability to meet the above requirement.
