- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulations (FAR) Subpart 12.6, in conjunction with FAR 13.5, as applicable, and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Offers are being requested and a written solicitation will not be issued. VA580-10-RQ-0086 is issued as a Request for Quotation (RFQ). The combined synopsis/solicitation will result in contract and corresponding purchase order. This requirement is available as brand name full and open competition for commercial items under the North Atlantic Industrial Classification System (NAICS) 339112; the business standard size is 500 employees. Each line item must have a description and pricing information. The total price offer shall include all charges (shipping). This procurement is for a firm fixed price contract.Scope of Contract: The contractor shall provide the following cardiac monitoring equipment: (quantity= 6 of each line item) 0001 Vigilance II CCO/Sv02 Monitor(VIG2),0002 Optics Module Cable (OM2),0003 Patient Cable(70CC2),0004 Monitor Roll Stand(VIG2S),0005 Adapter Plate (VIG2AP),0006 Total Protection Service (2 years)(VIG2TPSRV12) Government reserves the right to increase the quantity at same price by September 30, 2010. Equipment and Attachments will be delivered FOB: Destination to: Chief, A&MMS, Michael E. DeBakey VA Medical Center, Receiving Warehouse Building 100, 2002 Holcombe Blvd., Houston, Texas 77030-4298. Requested Delivery Date: 15 days After Receipt of Order. The provision and clauses incorporated into the solicitation document are those in effect at the time of publication. Provisions and clauses incorporated by reference have the same force and effect as if they were given in full text. This combined synopsis lists one or more clauses by reference with the same force and effect as if they were given in full text (FAR 52.252-2). The provisions at FAR 52.212-1, Instructions to Offerors-Commercial Items, apply to this acquisition with the following addenda to 52.212-1, 52.204-6 (Data Universal Numbering System (DUNS) Number to the provisions of the standard provisions. In accordance with Federal Acquisition Regulation FAR 52.212-2 Evaluation-Commercial Items all offers will be considered for BEST VALUE. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers. (1) Technical capability: ability of vendor to provide all Edwards LifeSciences equipment in accordance with model numbers given in description (2) Size Status: preference will given to socioeconomic business types following VA hierarchy; and (3) Price: Quote for each line item. Technical and price when combined are equally important. Failure to submit an offer on all items will render the offer non-responsive. Offerors shall complete FAR 52.212-3 Offerors Representations and Certifications-Commercial Items available for online registration at http://orca.bpn.gov or include a written copy with the quote submitted. The clause at 52.212-4 Contract Terms and Conditions-Commercial Items, applies to this acquisition, to include the following addendum to the clause: 52.204-7 (CCR registration) the successful offeror must be registered with the Central Contractor Registration (CCR) to be awarded a contract, you may register easily and quickly on-line at http://www.ccr.gov, 52.217-6 (Option for Increased Quantity), 52.232-34 (Payment by Electronic Funds), VAAR 852.233-70, (Protest Content), 852.236-76 (Correspondence), 852.237-70 (Contractor Responsibility) and 852.203-70(Commercial Advertising), 846.203-70 (Guarantee), 852.246-71 (Inspection, 852.211-70(Service Data Manuals). Clause at 52.212-5, Contract Terms and Conditions Required to Implement Statues or Executive Orders-Commercial Items, applies to this acquisition which shall include the following clauses by reference are applicable to this requisition.
