- REQUEST FOR PROPOSAL Physician Services At Sacramento Job Corps Sacramento Job Corps is seeking a subcontractor to provide on-site physician services to our students that range in ages of 16 to 24 years old. The physician will be required to work an average of 20 hours per week for 48 weeks per year. The days that services are needed are eight (8) hours every Wednesdays and Thursdays and four (4) hours every Fridays. This contract will be in effect from July 5, 2010 through June 30, 2011, with the possibility of two (2) one-year extensions. Necessary bidding documents and specifications may be obtained from the Purchasing Department, on or after April 16, 2010. Send an email request for proposal solicitation package to the buyer. Proposals must be received in the Purchasing Department to the attention of Jesse Rosario, Buyer, by 4:00 PM on May 10, 2010. Proposals may be sent via mail or email. Bidders should therefore review the format and clauses of this contract in order to prepare a response to provide PHYSICIAN services. In addition, Job Corps Manuals and Technical Supplements may be reviewed office of the Wellness & Health Manager. The Sacramento Job Corps Center reserves the right to reject any or all proposals or waive formalities in the bidding process.
