- DESCRIPTION:In accordance with FAR Part 10, Market Research, this is a sources sought to determine the availability of potential sources/vendors having the skills and capabilities necessary to perform/provide the stated requirements for Anesthesiologists services. All interested contractors are invited to provide information to contribute to this market survey/sources sought including, commercial market information. This is not a solicitation announcement. This is a sources sought synopsis only. Questions will not be entertained at this time. Respond only to requested information and questions below. The purpose of this synopsis is to gain knowledge of potential qualified sources and their size classification (Hubzone, 8(a), small, small disadvantage, service disabled veteran owned small business, or large business) relative to NAICS 621111 (size standard of $10.0 Million). Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. REQUIREMENT: The Department of Veterans Affairs Medical Center, 4100 W. Third Street, Dayton, OH, 45428-9000 is seeking sources (firms/contractors) for a potential requirement for Anesthesiologist services with duration of approximately three years; to furnish 1 Anesthesiologist for every 2 Certified Registered Nurse Anesthetists (CRNS's) up to five Anesthesiologists based on full time equivalent (FTE) Monday through Friday from 7 am to 3:30 pm and 1.5 FTE for Called-In Rate (Pager Week Nights and Weekends) Furnish up to 1.5 FTE (Week Nights-Monday - Friday from 7 p.m. to 7 a.m. for a total 60 hours / Week; Called-In Rate (Pager Holidays) up to.1 FTE (10 Holidays/Year, total 240 Hours/Year, 7 A.M. to 7 a.m.). The Anesthesiologists shall be available to perform the requirements of this contract twenty-four hours daily, seven days a week, three hundred sixty five days a year, at the firm fixed prices. The Contractor shall provide all labor and qualified personnel to perform all work. The Contractors submitting responses to this sources sought notice must provide consistent and knowledgeable staff based on the requirement stated herein. GENERAL PRIVILEGES FOR CONTRACT EMPLOYEES ARE AS FOLLOWS: 1.Cardiothoracic Surgery Anesthesia. 2.Vascular Surgery Anesthesia 3.Orthopedic Surgery Anesthesia 4.Genitourinary Surgery Anesthesia 5.General Surgery Anesthesia 6.Gynecological Surgery Anesthesia 7.Ear, Nose and Throat Surgery Anesthesia 8.Ophthalmological Surgery Anesthesia 9.Neurosurgical Anesthesia 10.Plastic Surgery Anesthesia 11.Podiatric Surgery Anesthesia 12.Management of cardiac and respiratory resuscitation problems, including participation in the "Code" or "Code 99" team. 13.Insertion and use of invasive monitoring lines-arterial, central venous and PA catheters. 14.Anesthesia services for procedures outside the Operating Room including lithotripsy, neuro-imaging and radiological procedures, cardiac catheterization laboratory and gastrointestinal studies, ECTs and Cardioversion. 15.Maintain educational curriculum as direct by Chief, Anesthesiology Service or his/her designee. OTHER SERVICES TO BE PERFORMED INCLUDE, BUT ARE NOT LIMITED TO: 1. Evaluation of patients pre-operatively 2. Administration of anesthetic agents and conduction of supportive therapy. 3. Undertake post-operative therapy referable to anesthesiology. 4. Furnish technical and/or professional direction of resident physicians and nurse anesthetists employed by Dayton VA Medical Center 5. Furnish consultations referable to anesthesiology. 6. Furnish emergency call services to the Dayton VA Medical Center Emergency call service is defined as the availability of and provision of anesthesiologist services required by the Dayton VA Medical Center during the periods outside of normal working hours and on weekends and holidays. Any "on-call" provider must be available for "in-house" (in person) consultation within 60 minutes of initial contact and request for anesthesia services by the "in-house" CRNA or Dayton VAMC staff physician. 7.Clinical Support for coverage of the pre-operative assessments, post operative visits and acute pain management. 8.Perform peer reviews as requested. 9.Provide monthly reports of clinical schedules to Chief, Anesthesiology Service or his/her designee 30 days prior. 10.The Contractor's anesthesiologists shall furnish normal work-day service and on call coverage to provide for all emergency procedures, to include elective cases outside normal work hours. Emergency call service is defined as the availability of and provision of all anesthesiologist services required by the Dayton VA Medical Center during time periods outside of normal working hours, to include weekends and holidays. An attending anesthesiologist must be available for coverage from 3:30 PM each weekday, to 7:00 AM the next morning, Monday through Friday and for weekend coverage from 3:30 PM on Friday until 7:00 AM on Monday or 7:00 AM Tuesday if Monday is a Government holiday 11.While "on-call" Monday through Friday, the anesthesia provider will remain in the Medical Center until all cases are completed and patients discharged from the post anesthesia recovery room. Any "on-call" provider must be available for "in-house" (in person) consultation within 60 minutes of initial contact and request for anesthesia services by the "in-house" CRNA or Dayton VAMC staff physician. 12.Clinical Coverage Review will include a description of the monitoring procedures used by the Dayton VA Medical Center to ensure contract compliance will be demonstrated through making sure that patient's surgeries were not canceled or delayed as a result of the contractors' anesthesiologists being unavailable, surgical package data, minutes of meetings, or other appropriate records, that the service called for under the contract have been received by the Contracting Officer's Technical Representative (COTR) who is responsible for verifying contract compliance. After contract award, any incidents of contractor non-compliance, as evidenced by monitoring procedures, shall be forwarded immediately to the contracting officer in writing. 13.Anesthesia will be delivered either by the attending physician or with the attending physician providing professional direction to the CRNA's and to house staff levels defined in VHA Manual M-2, Part 1, Chapter 26, "Supervision of Postgraduate Medical, Surgical, Dental Optometry and Podiatric Residents." Manual M-2 is available for viewing in the office of the Dayton VA Medical Center Chief, Medical Officer. Evidence of provision of professional direction and supervision to the residents will be demonstrated through the following: a.Pre-operative Note: A pre-operative anesthesia note will be written and signed and a consent for anesthesia obtained from every patient. Immediately pre-induction a re-evaluation and examination should be performed and documented by the attending anesthesiologist on every patient before he/she undergoes anesthesia. b.Intra-Operative Note: The attending anesthesiologist will write and sign the note discharging the patient form the post anesthesia recovery room, or the anesthesia record when the patient is discharged from the operating room to the ICU or ward. 14.The contractor shall provide timely completion of all required clinical documentation per policies and procedures. 15.The contractor shall insure that contract physicians participate in continuous quality improvement programs in the organization as described in VHA Handbook 1100.19. In addition, the services to be performed by the Contractor shall be done in accordance with VA Policies and procedures, Joint Commission on Accreditation of Healthcare Organizations (JCAHO), Federal and State Laws, and the Regulations of the Medical Staff By-Laws of the Dayton VA Medical Center 16.The contractor should thoroughly review the specifications and become familiar with the areas of coverage in order to become fully aware of the scope of the requirements. Failure to understand the contract requirements shall not relieve the Contractor form performing in accordance with the meaning and intent of the specifications. The Government will not reimburse costs that are not incorporated into the Contractor's price. 17.The contractor shall be responsible for the administration and management of all aspects of this contract and the health plan covered. This includes responsibility for all contractor employees, subcontracts, agents, and anyone acting for or on behalf of the contractor. (15) Provide company management capabilities and skills (16) Give a brief description of ability to provide 24 hour, 7 days a week, IR services (17) What is the number of readily available physicians that your firm employs? (18) What is the maximum number of employees your firm has employed within the last three years? (19) What is your firm's contingency plan in the event that your physicians are on leave, dismissed, or find employment elsewhere? (20) Would you need to subcontract to fulfill the requirements stated herein? If so, state with which firm and a point of contact for that firm. (21) Provide your encryption capabilities such as PKI. (22) What are your firm's customary practices regarding customizing, modifying or tailoring of items to meet customer needs and associated costs? Responses to this sources sought synopsis are not considered adequate responses to the solicitation announcement. All interested offerors will have to respond to the solicitation announcement in addition to responding to this sources sought announcement. Responses to this sources sought is not a request to be added to a prospective bidders list or to receive a copy of the solicitation. Telephone and faxed responses WILL NOT be accepted nor responded to.
