Sources Sought and Request for InformationVeterans Health Administration (VHA) Health Risk Assessment Acquisition and Integration The Department of Veterans Affairs (VA) is seeking sources and information for system integration of an online Health Risk Assessment (HRA) tool for use by patients and clinicians within the Veterans Health Administration. The purpose of this notice is to gain knowledge and information for the purposes of market research. Information gained by this request may or may not result in a subsequent solicitation. This Sources Sought (SS) Notice and Request for Information (RFI) is for planning purposes only and shall not be construed as a Request for Proposals (RFP), Invitation for Bid (IFB) or as an obligation on the part of the Government to acquire any services. Responses to this notice shall not serve as proposals, bids, or offers, which could be accepted by the Government to form a binding contract. The VA will not accept unsolicited proposals related to the subject of this notice. The responder must clearly identify any information it considers sensitive or proprietary. All information received in response to this notice that is marked Proprietary will be handled accordingly. Responses will not be returned. Responders are solely responsible for all expenses associated and incurred by responding to this notice. The Government reserves the right to determine how it should proceed as a result of this notice Background: The VHA provides the national healthcare system for United States' Veterans. These services are provided by 21 Veterans Integrated Service Networks (VISNs), administering 154 full-service hospitals, psychiatric facilities, community living centers, Veteran centers, research facilities, rehabilitation facilities and 675 Community-Based Outpatient Clinics. Comprehensive health care is provided through primary, tertiary, and long-term care in the areas of medicine, surgery, psychiatry, physical medicine and rehabilitation, neurology, oncology, dentistry, geriatrics, spinal cord injury, women's health and extended care. The VHA uses a decentralized electronic medical record (EMR) system within the Veterans Health Information Systems and Technology Architecture (VistA) and its next generation "HealtheVet". The Computerized Patient Record System (CPRS) is the graphical user interface through which VA staff access an individual's electronic medical records within VISTA. Further, the VHA has an online patient portal called "My HealtheVet" (www.myhealth.va.gov ). My HealtheVet is an Internet-based program that creates a new, online environment where veterans, family, and clinicians come together to optimize veterans' health care. It provides trusted information, online services, limited health record access, and messaging between veterans and clinicians. The VHA is seeking information to plan for a future acquisition of a patient-entered Health Risk Assessment tool, integrated into My HealtheVet and the VHA's current electronic medical record. Health Risk Assessments (HRA) are instruments that collect information from individuals through the use of structured questionnaires. HRAs often provide tailored feedback to the individual about his or her health status, with specific recommended steps to improve health. Core features of an HRA include assessment of personal information, past medical and surgical history, medications, family history, health behaviors (use of tobacco, alcohol, diet, physical activity, etc.), and receipt of preventive care (screenings and immunizations, etc.). Elements also desirable include information of self-care management for common chronic conditions such as diabetes and heart disease, health risks relevant to the elderly (such as fall risk), and readiness-to-change for health behaviors that increase patient risk for disease and/or injury. Requested Information: Respondents shall provide a general capabilities statement to address the information requested below. Based on an initial review of the capabilities statement submitted, respondents may be asked to give an oral presentation that includes a real-time demonstration of the HRA product and capabilities. 1.Information about the content of the HRA Instrument including the following: a.Clinical domains for questions and feedback to patients covered on the HRA b.Items used to assess various clinical domains, including development history, source of preventive care items in the clinical domains, and/or reliability/validity for items used on the HRA instrument c.Reading level of items used on HRA and information from formal usability testing of items d.Decision algorithms used to present tailored items to individuals and decision algorithms used to generate feedback to individuals, including development history and/or scientific justification for selected algorithms e.Patient feedback based on the responses to HRA items, including reading level and results from formal usability testing of feedback language used f.Length of instrument and average time to complete and/or other user statistics g.A detailed description of the capability for modifying existing HRA question items, branching logic, decision algorithms, as well as adding new question items/logic/algorithms and modifying responses and feedback to users to ensure content is appropriate and targeted for the VHA user population and whether non-technical staff can make any modifications. h.Description of the capability to generate an individual patient-level summary of HRA responses for the patient's clinical provider. i.Description of previous applications of HRA instrument in clinical settings and integration into electronic medical records or personal health records. 2.Information about the HRA user interface design. This includes the following: a.Overall look and feel of the graphical user interface and how individuals progress through the HRA from start to finish. b.Capabilities for the individual completing the HRA, such as printing, saving, and modifying individual HRA item responses. c.Results of usability testing of the graphical user interface with patients d.Compliance with Section 508 requirements (http://www.section508.gov/) 3.Information about the technical specifications and operating requirements for deploying the HRA within the VHA operating environment in real-time, including but not limited to the following: a.Hardware requirements for optimum functioning of the HRA b.Operating environment and software requirements for optimum functioning of the HRA c.Use of interoperability standards recognized by the Secretary of Health and Human Services, including interoperability with the Nationwide Health Information Network (NHIN). (http://healthit.hhs.gov/portal/server.pt?open=512&mode=2&cached=true&objID=1142) d.Use of industry standard protocols with regard to network, operating system, database application, development languages, and hardware. Specifically detail whether proprietary single-source technology or exclusive use of a singular protocol is used. e.Capability for integration of the HRA into existing VHA systems, including MyHealtheVet and VISTA/CPRS. This includes the ability for: i.Launching the HRA tool from within the My HealtheVet portal ii.Individual receipt of HRA feedback from within the My HealtheVet portal iii.Clinician receipt of HRA feedback from within VISTA/CPRS iv.Real-time bidirectional flow between My HealtheVet and VISTA/CPRS to allow for pre-population of relevant HRA items based on existing data in VISTA/CPRS (e.g., birthdate, blood pressure, etc.) and importing of completed HRA results into computable data fields within relevant VISTA/CPRS packages, wherever possible. f.Capability for generating population-level reports at the facility, VISN, regional, and national levels. 4.Information about capabilities for managing the entire acquisition, integration and future maintenance of a fully integrated HRA tool within the VHA system. Specifically: a.Project management and experience with large scale IT development/integration projects b.Use of subcontractors c.Capabilities for ongoing development, maintenance, and support of the integrated HRA tool within the VHA operating environment.
