- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6. This announcement constitutes the only solicitation and a written solicitation will not be issued. This synopsis, NAICS code 541611, identified as REQ1069869, is to notify contractors that the government intends to issue a Purchase Order in accordance with FAR Part 13.106 for the following statement of work, under the simplified acquisition procedures. Prospective offerors are responsible for downloading the solicitation and any amendments. It is the offeror's responsibility to monitor the FedBizOpps website for the release of any amendments to this solicitation. Hard copies will only be provided to individuals eligible under the Americans with Disabilities Act and Rehabilitation Act upon request. The Government reserves the right to award a contract without discussions if the Contracting Officer determines that the initial offer(s) is/are providing the Best Value and discussions are not necessary. Statement of Work CFSAN 360-Degree Survey Administration Background As part of its Organization Development Initiative, the Center for Food Safety and Applied Nutrition (CFSAN) formed a working group to identify managerial values and behaviors that they agreed to espouse and believe are the foundation of the CFSAN Leadership Philosophy Statement. Create as positive a work culture as possible to support continuing higher performance by all members of the Center. CFSAN’s Organizational Development Manager created a 360-degree survey to evaluate the extent to which managers are demonstrating these values and behaviors. The survey has been administered to CFSAN’s Leadership Team. The Center Director wants to extend administration of the survey to all remaining CFSAN managers. Scope Contracting shall administer the 360-degree evaluation to approximately 175 CFSAN managers. Online administration of the survey by a third party is required to maintain confidentiality and anonymity of raters. The surveys will be administered in groups from time to time over 12 to 18 months. The contractor shall perform the following services: •Sending participants and raters an e-mail with a link to take the survey. •Monitoring, and when necessary, encouraging participation. •Tracking usage and providing update reports. Deliverables Prepare and submit a monthly summary report indicating the dates and number of hours worked, and a description of the work performed. The monthly report shall be submitted to the Project Manager and Contracting Officer Representative within 15 business days after the end of each month. The report shall show the title and contract number on the cover page. A feedback report for each participant with numeric and qualitative feedback and highlights of highest and lowest scores. One printed copy (color, bound, and sealed) will be provided as well as an electronic version in PDF format. SECURITY REQUIREMENTS – N/A ASSUMPTIONS AND/OR CONSTRAINTS - N/A The following criteria and weights shall be used by CFSAN to evaluate Contractor proposals: Past Performance - 50 points The offeror shall demonstrate that they have successfully performed activities of similar size and scope as those identified in the statement of work within the past five years. This shall be done by describing current and past work of a similar or identical nature in such a manner that an evaluation can be made of performance history and the relevance of this experience to the requirements of the solicitation. Corporate Experience - 40 points The offeror shall clearly demonstrate that they have the technical expertise necessary to perform the tasks outlined in the statement of work. Qualifications of Personnel - 10 points The offeror will demonstrate that key personnel have the requisite experience and qualifications to satisfy CFSAN’s requirements. Offeror must provide evidence of related education, knowledge and experience for proposed personnel in the activities in the statement of work. Total Possible Points = 1OO Points Vendors must be registered in the Central Contractor Register (CCR) prior to the award of a contract. You may register by going to www.ccr.gov. You will need your DUN and Bradstreet number and banking information. The anticipated award date is March 31, 2010.
