Description
This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR 12.6, with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-38 as corrected. This solicitation is unrestricted. The NAICS code is 621340, with a size standard of $7 million. All services shall be performed at the VA Sierra Nevada Healthcare System (VASNHCS), 1000 Locust Street, Reno, Nevada 89502. Scope of Work. 1.1 The contactor shall provide a Certified Recreational Therapist to perform recreational therapy services at the VA Sierra Nevada Healthcare System (VASNHCS), 1000 Locust Street, Reno, Nevada 89502. 2. Specific Contractor tasks include, but are not limited to: 2.1 The Contractor shall provide recreational therapist services from 10:00 am to 6:30 pm, Tuesday through Saturday, excluding holidays. No overtime is authorized. 2.2 Administer and document a Recreational needs assessment and Minimum Data Set (MDS) upon admission to the Community Living Center (CLC), and update assessment quarterly and annually for all residents. 2.3 Conduct therapy sessions to improve patients' mental and physical well-being. 2.4 Observe, analyze, and record patients' participation, reactions, and progress during treatment sessions, modifying treatment programs as needed. 2.5 Plan, organize, direct and participate in treatment programs and activities to facilitate patients' rehabilitation, help them integrate into the community and prevent further medical problems. 2.6 Develop treatment plan to meet needs of patient, based on needs assessment, patient interests and objectives of therapy. 2.7 Prepare and submit reports and charts to treatment team to reflect patients' reactions and evidence of progress or regression. 2.8 Obtain information from medical records, medical staff, family members and the patients themselves to assess patients' capabilities, needs and interests. 2.9 Counsel and encourage patients to develop leisure activities. 2.10 Instruct patient in activities and techniques, such as sports, dance, music, art or relaxation techniques, designed to meet their specific physical or psychological needs. 2.11 Confer with members and participates of Interdisciplinary Treatment Team to plan and evaluate therapy programs. 2.12 Encourage clients with special needs and circumstances to acquire new skills and get involved in health-promoting leisure activities, such as sports, games, arts and crafts, and gardening. 3. Qualifications. 3.1 The Contractor will be responsible for ensuring that the recreational therapist providing services under this contract is fully trained and completely competent to perform the required services covered by this contract. The recreational shall be/have at a minimum: 3.1.1 Bachelor degree in therapeutic recreation from an accredited university. 3.1.2 Certification as a Recreational Therapist from the National Council for Therapeutic Recreation Certification (NCTRC) or Activities Director. 3.1.3 Minimum of 1 year experience within the past 2 years in activities or programs for individuals and groups that require application of knowledge of the concepts, principles, and practices of recreational therapist and involve the use of therapeutic approaches to maintain the physical and/or mental health or to achieve the physical and/or mental rehabilitation of persons.
