Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is a Request for Proposal (RFP). This procurement is issued unrestricted under the NAICS code 336992 with a small business size standard of 1,000 employees. A Statement of work is incorporated in the attached Solicitation and will be made a part of any resulting award. Please see attached forms for Past Performance Evaluations. To forms are provided to complete the evaluation: 1) Performance Rating Form; and 2) Supplemental Comment Form. Offeror shall prepare and submit both forms to prior client(s). This includes correspondence explaining the requirement to obtain independent evaluation of prior contract performance, and the Past Performance Questionnaire for use in evaluating Past Performance. Completed questionnaires shall be mailed directly to: US Property & Fiscal Office New Jersey so as to arrive not later than the closing date for recent proposal shown on SF33. Reference packages may also be faxed to 609-562-0229 or emailed to Kathy.m.hines@us.army.mil. NOTE: Offeror must not review the Past Performance Questionnaire prior to submission. They must be submitted by the clien/evaluator directly to the Contracting Office. Each offeror shall indicate if credit card (VISA) is an acceptable form of payment. Award will be made to the offeror determined to be the best value to the government based on the attached Evaluation Criteria. To determine Best Value, all requirements of the Statement of Work must be presented. The following minimum requirements must be met: (1) The quote must provide all services as per Statement of Work. (2) A completed price sheet must be provided. The Government intends to evaluate offers and award a contract without discussions with offerors. Therefore, the offerors initial offer should contain the offerors best terms from a price and technical standpoint. All questions shall be submitted in writing and received prior to bid closing.
