Description
Air Force Form 813 pages 1 and 2. Performance Work Statement for Basewide Tree Service This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) Solicitation F4D3D90013AQ01 is issued as a request for proposal (RFP). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-38. (iv) This procurement is being issued as a 100% Small Business Set-Aside under NAICS code 561730 and small business size standard $7.0M. (v) 0001: Tree Maintenance, Trimming, Removal -per Hourly Labor Rate 0002: Tree Trimming & Removal near Powerlines -per Hourly Labor Rate 0003: Removal of Stumps/Grinding -per Hourly Labor Rate 0004: Herbicide -per Hourly Labor Rate Tree trimming to ensure a healthy natural or manicured appearance; trees shall be trimmed to ensure adequate safety clearnances from sidewalks, structures, roadways and powerlines, IAW commercial and ANSI standards and Air Force Form 813 (see attached Performance Work Statement and Form 813). Trees identified for removal. Trees shall be cut to ground level and removed IAW ANSI standards. If power line disruption is required, Quality Assurance Personnel shall coordinate with 30th Civil Engineering Electric Shop at least 24 hours prior (see attached Performance Work Statement and Form 813). Contractors may be tasked to remove stumps. Stump removal shall be accomplished using industry standards. Grind removed stumps and disposal IAW 1.4 Waste materials/Weigh-In and Disposal of Tree Materials. Holes shall be filled with topsoil and seeded/sod to match the adjacent area (see attached Performance Work Statement and Form 813). Apply chemecial herbicide to prevent re-growth of trees (see attached Performance Work Statement and Form 813). Wage Determination 77-0727 (Rev.-37) Forestry and Land Management Services is applicable (vi) Period of Performance is 1 March 2010 to 28 February 2011, to 30th Civil Engineering Squadron, Bldg 11439 Rm 130 Vandenberg AFB 93437-6011. (vii) The provision at 52.212-1, Instructions to Offerors--Commercial, applies to this acquisition. (viii) 52.212-2, Evaluation--Commercial Items applies. Quotes will be evaluated based on Price, Technical Capability, & Best Value. (ix) Offerors must include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications--Commercial Items, or complete on ORCA website: http://orca.bpn.gov/ with its offer. (x) Clause 52.212-4, Contract Terms and Conditions--Commercial Items, applies to this acquisition.
