- SCHEDULE OF SUPPLIES AND SERVICES This procurement action is to obtain wheelchair ramps and related installation/configuration for three (3) Government owned vehicles for the Department of Veterans Affairs (VA) Greater Los Angeles Healthcare System, 11301 Wilshire Boulevard, Los Angeles, CA 90073. The Government intends to award a firm, fixed price contract for this service. Line item price shall include all equipment, parts, labor, freight, delivery, and materials related to this contract. Unit Price Total 1Wheelchair Ramp and Related Installation, per specifications. 3 $ $ This is a combined synopsis/solicitation for the procurement of wheelchair ramps and related installation/configuration of three (3) Government owned vehicles prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will NOT be issued. Provisions and clauses are incorporated through Federal Acquisition Circular 05-38. This procurement is utilized as a Small Business Set-Aside. The solicitation number for this Request for Quotation is VA-262-10-RQ-0137. The North American Industry Classification System Code (NAICS) is 441310. DESCRIPTION OF SUPPLIES/SERVICES: The VA Greater Los Angeles Healthcare System has a requirement for wheelchair ramps and related installation/configuration of three (3) Government owned vehicles. Significant characteristics of equipment components are as follows: 1. SCOPE: The contractor shall provide wheelchair ramps and related installation/configuration for three (3) Government owned Dodge Grand Caravans. 2. EQUIPMENT/ INSTALLATION/ CONFIGURATION: In each vehicle the contract shall: -Provide and install ADA compliant 54'' manual fold-out, swing away wheelchair ramp. -Provide and install 56'' high ramp entry door ramp entry and left door. -Provide and install two sets of four (4) point lap and shoulder restraints and related restraint track right front and center locations. -Install two wheelchair stations next to driver in front and wheelchair station in second row. Leave last row of bench seats in the vehicle. -Provide and configure 14'' lowered stainless steel floor with rust proofing. -Provide OEM Dodge SER load leveling/height control rear suspensions. -Provide and install back up alarm. -Provide commercial grade flooring in the color black. -Provide interior lighting package (ramp dome lighting) -Provide and install grab handles (right A pillar, both B pillars). - Emergency rear exit cable and sign - Install removable seat next to driver - Remove second row of seats (retain and return for the Government) 3. WARRANTY: Contractor shall provide a full manufacture warranty on all parts and installation. 4. DELIVERY: Delivery of the Government owned vehicles to and from the contractor facility shall not be billed separately, but shall be included into the line item cost. The contractor shall ensure that delivery is accomplished by a flatbed method of towing. Delivery shall be coordinated with the Contracting Officer's Technical Representative. The vehicles are currently located at the VA Greater Los Angeles Healthcare System, 11301 Wilshire Blvd. Los Angeles, CA 90073. Inspection and acceptance of the vehicles shall occur at the time of delivery. PROVISIONS/CLAUSES: Offeror shall submit a complete copy of provision FAR 52.212-3, Offeror Representation and Certification- Commercial Items (AUG 2009). CONTENT OF OFFERS: Offerors responding to this announcement shall provide all information contained in the Federal Acquisition Regulation (FAR) Clause 52.212-1, Instructions to Offeror-Commercial Items. As stated in this clause, the Government intends to evaluate offers and award a contract without discussions with offerors. Therefore, the offerors initial offer should contain the offeror's best terms from a price and technical standpoint. However, the Government reserves the right to conduct discussion if later determined by the Contracting Officer to be necessary. Contractor shall submit the following items in its proposal(s): 1.Schedule of Prices 2.References: name and phone numbers of three (3) customers who have purchased, leased, or rented or purchased equivalent systems. 3.Brochures on actual unit proposed (For equivalent equipment, must include component specifications). 4.Warranty Information 5.Complete copy of provision FAR 52.212-3, Offeror Representation and Certification- Commercial Items (AUG 2009). 6.Company's DUNS number.
