- Organizational Conflict of Interest (OCI) Brief, 7 Jan. 10 (Pre-proposal Conference) Amend. 5 - Section L, Attachment 5 - Organizational Conflict of Interest (OCI) Form Amend. 5 - Section L, Attachment 2- Past Performance Questionnaire (PPQ) Form Amend. 5 - Section L, Attachment 2- Past Performance Questionnaire (PPQ) Cover Page Amend. 5 - Section L, Attachment 1- Past Performance Information (PPI) Form Amend. 5 - Section L, Attachment 1- Past Performance Information (PPI) Cover Page Amend. 5 - Section L AMENDMENT #5 The purpose of this amendment is to extend the due date of all Proposal Volumes (I, II, III, IV & V) and all tabs to 16 February 2010. This extension is due to the Government’s clarification of the MiDAESS Organizational Conflict of Interest (OCI) position (Reference the attached OCI Brief from the 7 Jan. 2010 Pre-proposal Conference) which may have an impact on current teaming arrangements for potential Offerors. The Government desires to maximize competition and to allow industry to re-align their Prime/Sub-contracting teaming arrangements, if necessary, and have adequate time to prepare proposals. Delivery of these proposal volumes shall also be IAW Section L-05.3 and Section L Table L-05-1 as revised by this amendment. ***** Offerors are reminded to review and follow proposal delivery instructions at Section L-06.4. UPS or FedEx delivery to the post office box or Schriever AFB Visitor’s Center is NOT authorized by the Contracting Officer. Delivery to the Schriever AFB Visitor’s Center is HAND DELIVERY ONLY by the Offeror; hand delivery must be coordinated with the Contracting Officer no later than 24 hours in advance of the proposal due date. Hand delivered proposals will ONLY be accepted on the proposal due date, 16 Feb 2010. ***** Offerors are hereby instructed to replace Section L; Section L - Attachment 1, Past Performance Information (PPI); Section L - Attachment 2, Past Performance Questionnaire (PPQ); and Section L – Attachment 5, Organizational Conflict of Interest (OCI) Disclosure Form with the amended versions. All revisions in these documents are related to delivery dates only. Offerors may submit revisions or updates to previously submitted Volume IV Past Performance Tabs 1, 2, 3, 4, 5, and 6 no later than 16 February 2010, if needed. Submission of Volume IV Past Performance Tab 3: Past Performance Questionnaires (PPQ) with previous delivery dates is acceptable as the content of the document has not changed. Only the due dates on this document were affected by this amendment. Note: The Industry Day #3 (Pre-proposal Conference), 7 Jan. 2010 Questions & Answers and Briefing Package were provided as an UPDATE to the Final Offeror’s Library and will not be posted separately to FedBizOpps. Offerors are required to acknowledge receipt of all amendments by filling in Block 14 of the SF 33. (Reference instructions at Section L-04.8)
