- ATTERBURY JOB CORPS CENTER REQUEST FOR PROPOSAL PHYSICIAN SERVICES Atterbury Job Corps Center and IndyPendence Job Corps Center is seeking proposals for a subcontract for our Physician services. Atterbury Job Corps Center is a 550 student residential training facility and IndyPendence Job Corps Center is a 100 student non- residential training facility. Atterbury and IndyPendence Job Corps Centers are a federally funded Educational and Vocational Training Program that provides services to students between the ages of sixteen (16) and twenty-four (24). The medical services to be provided under this subcontract shall conform, at a minimum, to health services specified in the Job Corps Federal Regulations (Title 20 Code of Federal Regulations, Part 638.5 10), in the Job Corps Policy and Requirements Handbook (PRH), Chapter 6.10 Ri and applicable Technical Assistance Guides (TAGs). To accomplish these ends, the Center Dentist shall: Provide medical services for a maximum of twenty-six (26) hours per week for the Physician, fourteen (14 hours per week) and Nurse Practitioner, twelve (12 hours per week). Of the twenty-six (26) hours per week, twenty-two (22) hours per week shall be provided on-site at the facilities located at the Center Wellness Services Clinic in Edinburgh, Indiana and four (4) hours per week will be provided on-site at the facilities located at the IndyPendence Center in Indianapolis, Indiana. Center staff will provide clinical services in emergency situations only, except as provided in written standing orders. The successful bidder will be selected based upon a combination of professional experience, quality of technical services and reasonableness of hourly rate. Bidders are placed on notice that this is a competitive process and cost is an important factor. All interested parties should submit completed bid packet and a written response via email ( gushwa.patty@jobcorps.org ) or by fax, 812-314-6290, or by USPS or other acceptable delivery methods, by 4:00 P.M., Friday, January 22, 2010.
