- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. W81K00-10-T-0026 is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-37. This acquisition is small business set aside under NAICS code 611430, Size Standard 7.0M. FAR provisions and clauses in this solicitation may be viewed at http://www.arnet.gov/far/. DFARS clauses may be viewed at http://www.acq.osd.mil/dpap/dars/dfarspgi/current/index.html. The requirements in this solicitation: Hospital First Receiver Decontamination Operations Training. QTY 12 Classes Period of Performance 20 Jan 2010- 30 Sep 2010. Quote should include prices for a base year and four option years. Performance Work Statement: 1. Scope: Non-Personal Services: Contractor shall perform Hospital Decontamination Operations Training Courses at Brooke Army Medical Center (BAMC). Emergency Healthcare Provider Roles course is designed to increase the level of knowledge that Emergency Providers face when patients have become exposed to hazardous substances includingHazardous Materials (HazMat) and Weapons of Mass Destruction(chemical, biological, radiological, or nuclear contaminants). Participants should gain the knowledge of proper medical approach to mass casualty incidents, hospital decontamination responsibilities, personal protective equipment, personal and patient decontamination, and contamination avoidance. 2. Contactor shall: 2.1 Provide one or more qualified instructors for each of the scheduled training sessions. 2.2 Conduct a pre-course survey-Enviromed Resources will contact the BAMC Point of Contact (POC) at least seven (7) days prior to the initial scheduled training; to evaluate the course training areas and any other pertinent course delivery information. 2.3 Will provide classroom materials for each participant, including a course manual and other materials required for training. 2.4 Will provide Audio Visual equipment necessary for the course, unless otherwise coordinated with the BAMC POC. 2.5. Will provide Training Personal Protective Equipment (PPE), unless other arrangements have been made or if other equipment is available. 2.6 Will provide Certificate of Completion Training Certificates for all personnel who satisfactorily complete the training course. 2.7 Will provide Continuing Education Certificates for all personnel who satisfactorily complete the training course. 3. Host Medical Treatment Facility will: 3.1 Will provide a Point of Contact (POC) to assist with the coordination of each training session. 3.2 Will provide a suitable classroom for the selected number of participants. 3.3 Will provide an area to complete the Hands On portion of the training. This area should have water and proper drainage accessible for training operations. 3.4 Will be responsible for the selection of personnel and any medical clearances of the scheduled participants. Vendor must meet the minimum qualifications: At least 3 years experience in teaching OSHA programs in a healthcare environment. At least 2 years experience in healthcare operations. At least 3 years experience working or training within the Hospital Incident Command System. Instructors must be licensed Emergency Medical Technicians from the State of Texas or the National Registry. Instructors must possess documentation of the National Incident Management System 100HC, 200HC, 700a and 800b courses. Instructors must demonstrate competent instructional skills and knowledge of the applicable subject matter. Course must be currently approved as a Continuing Education Activity the Texas Nurses Association for continuing education hours to be awarded. Delivery date is 20-Jan-2010. Place of delivery and acceptance is Brooke Army Medical Center, Fort Sam Houston, TX, 78234. FAR provision 52.212-1, Instructions to Offerors Commercial, applies to this acquisition to include the following addenda: All offerors are cautioned that if selected for award, they must be registered with the Central Contractor Registration (CCR) program. The CCR can be accessed via the internet at www.ccr.gov/. Confirmation of CCR registration will be validated prior to award. Offers will be evaluated according to 52.212-2. Evaluation factors at 52.212-2 (a) conformance to minimum qualifications and (b) price. Vendors shall include a completed copy of FAR provision 52.212-3 as outlined therein.
