Description

Amendment 2 - Section L, Attachment 5 - Organizational Conflict of Interest (OCI) Disclosure Form Amendment 2 - Section L, Attachment 2 - Past Performance Questionnaire (PPQ) Form Amendment 2 - Section L, Attachment 2 Cover Page Amendment 2 - Section L AMENDMENT #2. The purpose of this administrative amendment is to extend the due date of the Organizational Conflict of Interest (OCI) Plan and Disclosure Forms, Volume III – Management Tab 3, (Reference Section L-09.3) to 25 January 2010. Additionally, the due date of Past Performance Questionnaires (PPQ), Volume IV – Past Performance Tab 3 (Reference Section L-10.3) is extended to 25 January 2010. Delivery of these proposal tabs shall also be IAW Section L-05.3 and Section L Table L-05-1 as revised by this amendment. Offerors are hereby instructed to replace the original Section L, Section L - Attachment 2, Past Performance Questionnaire (PPQ) Form, and Section L – Attachment 5, OCI Disclosure Form with the amended versions. All revisions in these documents are related to delivery dates only. Offerors are required to acknowledge receipt of all amendments by filling in Block 14 of the SF 33. (Reference instructions at Section L-04.8)
