- Addendum to Solicitation Standard Form 1449 Request for Quote The Social Security Administration (SSA), Region 5, has a requirement for contractor provided fraud investigation services for the Office of the Inspector General, Cooperative Disability Investigations (CDI) Unit in Chicago, IL. The contractor shall provide the necessary personnel, materials, services, equipment, and perform all tasks necessary for or incidental to providing the required services to investigate fraud against SSA's Title II and Title XVI disability programs and other Federal and State programs, including Title XIX, for SSA in Chicago, IL. The contractor provided personnel must have law enforcement (arrest) authority, as defined by the statutes and regulations of the State of Illinois. The contractor is required to provide all necessary equipment for its employees assigned under this contract including, but not limited to, vehicles, weapons, body armor, cellular phones, and etc. This acquisition is prepared in accordance with Federal Acquisition Regulation (FAR) Subpart 6.302-1. All contract awardees will be required to undergo a security background screening which will include completion of fingerprint cards before beginning performance under the contract. To be eligible for award, offeror must be registered in the Central Contractor Registration database prior to award. For registration information, go to the Central Contractor Registration website at http://www.ccr.gov. Offerors must have a Dun and Bradstreet (DUNS) number to begin registration. The period of performance will be from March 1, 2010 through February 28, 2015. The North American Industry Classification System Code (NAICS) is 922110. Request for copies of a solicitation will not be honored or acknowledged. Responses to this announcement must be in writing and submitted to the address cited at the beginning of this notice or by email to Babafemi.Littlejohn@ssa.gov. The due date for responses is Tuesday, December 31, 2009 at 3:00 PM, CST. Faxed responses shall not be permitted.
