- This is a bidding schedule for a base and 4 years. PLEASE SUBMIT BID FOR ONE FACILITY OR BOTH The contractor must provide health and fitness memberships designed to promote better health and physical fitness for the U.S. Army Corps of Engineers, Buffalo District employees. The contractor shall provide these services for up to 30 Corps employees at various locations in Ohio and New York. The contract shall initially be for one year with an option to renew for 4 additional years. The off-site fitness facility shall meet the following requirements: •1. Be within driving distance (25 miles) from each individual field office location: •a. Toledo, OH Office - 3906 North Summit Street, Toledo, OH 43611 •b. Oak Harbor, OH Office - 240 Lake Street, Unit D, Oak Harbor, OH 43449 •c. Orwell, OH Office - 33 Grand Valley Avenue, Orwell, OH 44076 •d. Auburn, NY Office - 7413 County House Road, Auburn NY 13021 •2. Have a full-time certified fitness director and complete fitness testing capabilities. •3. Upon request, provide a listing to participating employee members of other affiliated facilities throughout the United States that will allow visiting privileges while in travel status. •4. Provide shower and locker facilities for both men and women. •5. Provide a monthly and cumulative attendance record to the Corps each month that indicates how frequently each employee uses the facility. Attendance records will be sent to the Safety Office within seven days following the end of each month. •a. Safety Office address is: U.S. Army Corps of Engineers, Safety Office, 1776 Niagara Street, Buffalo, NY 14207 •6. Provide the option for Corps employees, at their own expense, to expand the Corps membership to include family members. Employee expense will be limited to the difference between the Corps membership fee and the expanded membership fee. •7. Provide the following facilities (facilities and equipment will be adequate for the size of the membership served): •a. A variety of strength training methods with both free-weight and stationary devices, e.g., Nautilus, Universal; •b. A variety of aerobic equipment for increasing cardio/vascular/pulmonary fitness, e.g., treadmills, elliptical runners, stationary bikes, stair machines; •c. Showers, lockers and change rooms; •d. Ensure the availability of professional staff to instruct and monitor employees in the safe/effective use of equipment. Information on healthy living classes must also be available. •8. Use of facilities at the affiliated locations within its organizational structure without additional cost to employees. PAYMENT: Payment will be made by government commercial credit card to the contractor after receipt of invoice at the end of each three-month period of the given contract year.
